摘要
目的观察舒筋洗外用颗粒对全膝置换后早期关节功能的疗效及对炎症因子的影响并探讨其作用机制。方法67例患者按随机数字表法分为观察组33例和对照组34例。对照组予塞来昔布、低分子肝素、持续被动训练(CPM)康复治疗;观察组在对照组基础上加用舒筋洗外用颗粒熏洗治疗。对比两组临床疗效、膝部肿胀值、关节活动度、视觉模拟量表(VAS)评分、美国特种外科医院膝关节评分(HSS)评分、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]变化。结果观察组总有效率为90.91%,高于对照组的70.75%(P<0.05)。治疗后两组膝部肿胀值、VAS评分及炎症因子(IL-6、TNF-α)水平均降低(均P<0.05),关节活动度和HSS评分均上升(均P<0.05);术后30 d,观察组的膝关节肿胀值、VAS评分、炎症因子(IL-6、TNF-α)水平低于对照组(均P<0.05),关节活动度和HSS评分高于对照组(均P<0.05)。两组患者均未见伤口感染、过敏性皮炎、深静脉血栓形成等不良反应。结论舒筋洗外用颗粒能减轻全膝置换术后肿痛,抑制炎症反应,有利于早期关节功能的康复。
Objective:To observe the clinical efficacy of Shujinxi External Application Granules on early joint function after total knee arthroplasty(TKA)and its effect on inflammatory factors.Methods:67 patients after TKA were randomly divided into observation group and control group,with 33 cases in the observation group and34 cases in the control group.The control group was treated with celecoxib,low molecular weight heparin,and continuous passive motion(CPM)rehabilitation therapy;the observation group was additionally treated with fumigation and washing with Shujinxi External Application Granules on the basis of the control group.The clinical efficacy,knee swelling value,joint range of motion,VAS score,HSS score,and inflammatory factors(IL-6,TNF-α)were compared between the two groups.Results:The total effective rate was 90.91%(30/33)in the observation group and 70.75%(24/34)in the control group,and the difference between the two groups was statistically significant(P<0.05).Over time,the knee swelling value,VAS score and inflammatory factor(IL-6,TNF-α)levels in both groups decreased(P<0.05),and the joint range of motion and HSS score increased(P<0.05).On the 30th day,the knee joint swelling value,VAS score,and inflammatory factor(IL-6,TNF-α)levels in the observation group were lower than those in the control group(P<0.05),and the range of motion and HSS score in the observation group were higher than those in the control group(P<0.05).Conclusion:Shujinxi External Application Granules can reduce swelling and pain after TKA,inhibit inflammatory response,and be beneficial to the recovery of early joint function.
