摘要
目的:探讨磁共振成像(MRI)表面小线圈在指间关节中的应用价值。方法:选取2019年1月~2020年12月本院类风湿性关节炎(RA)患者60例作为观察组,同期骨关节炎患者60例作为对照组。均采用MRI表面小线圈检查指间关节,对比两组诊断RA的临床价值。结果:观察组SNR、CNR高于对照组,差异有统计学意义(P<0.05);RA家族史、滑膜增厚、血管翳强化、骨髓水肿、指间关节积液情况对比,差异有统计学意义(P<0.05);采用偏相关性系数矫正工作性质、滑膜增厚、血管翳强化、骨髓水肿、指间关节积液情况后,SNR、CNR值仍与RA发生有关(P<0.05);SNR、CNR联合诊断RA的AUC值大于任一参数单独诊断(P<0.05)。结论:MRI表面小线圈检查指间关节参数与RA发生相关,在RA诊断中具较高应用价值,对治疗方案的制定有指导意义。
Objective:To investigate the application value of magnetic resonance imaging(MRI)surface coil in the interphalangeal joint.Methods:From January 2019 to December 2020,60 patients with rheumatoid arthritis(RA)in our hospital were selected as the observation group,and 60 patients with osteoarthritis during the same period were selected as the control group.The interphalangeal joint was examined by MRI surface coil,and the clinical value of RA diagnosis was compared between the two groups.Results:SNR and CNR in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).There were statistically significant differences in RA family history,synovial thickening,pannus enhancement,bone marrow edema and interdigital joint effusion(P<0.05).The results showed that SNR and CNR values were still correlated with RA occurrence(P<0.05)after the correction of working characteristics,synovial thickening,pannus enhancement,bone marrow edema and interphalangeal joint effusion(P<0.05).The AUC value of SNR and CNR combined diagnosis RA was greater than that of either parameter alone diagnosis(P<0.05).Conclusion:MRI surface coil examination of interphalangeal joint parameters is correlated with RA,which has high application value in RA diagnosis and guiding significance for the formulation of treatment plan.
