摘要
一只5月龄去势公猫,打斗后出现后肢瘫痪及排尿障碍。经X线检查未发现胸腰椎及髋关节的结构异常。西兽医推断存在腰骶部神经损伤,中兽医诊断为外伤引起的气血瘀阻,并伴有肾气虚损。首先使用地塞米松、利多卡因及氨苄西林钠进行封闭治疗,缓解疼痛并控制局部炎症。随后选择肾俞(BL-23)、膀胱俞(BL-28)、腰百会(Bai-hui)、二眼(Er-yan)、太溪(KI-3)、昆仑(BL-60)及涌泉穴(KI-1)进行针刺,同时用氦氖激光照射腰百会穴。经过3次针刺及激光治疗后,患猫行走与排尿功能完全恢复。针刺具有缓解疼痛、促进组织与神经修复的功能,激光具有扩散效应,仅需选择少量穴位进行辐照治疗。本案例结合使用局部封闭、针刺及激光治疗,取得了较好的效果。笔者通过本案例探讨猫后肢瘫痪的中西医结合诊疗思路,以期为此类疾病的临床治疗提供参考。
A 5-month-old castrated male cat suffered from nonambulatory paraplegia and urinary dysfunction after a fight.X-ray examination showed no structural abnormality of thoracolumbar spine and hip joint.Western medicine inferred that there was a lumbosacral nerve injury.While the diagnosis of traditional Chinese veterinary medicine was local Qi and blood stagnation caused by acute injury,combined with kidney-Qi deficiency.Firstly,the cat was treated with dexamethasone,lidocaine and ampicillin sodium in order to control the local inflammation and pain.Then we used acupuncture and He-Ne laser for treatment.BL-23,BL-28,Bai-hui,Er-yan,KI-3,BL-60 and KI-1 were selected as acupuncture points.And Bai-hui was also selected as laser radiation acupoint.After 3 times of treatment,the cat’s walking and urination functions recovered completely.Acupuncture can relieve pain,promote tissue and nerve repair.Laser therapy has a diffusion effect,only a small number of acupoints are needed for treatment.In this case,comprehensive treatment of local blocking,acupuncture and He-Ne laser have achieved good therapeutic effect.Through this case,the authors discuss the diagnosis and treatment ideas of integrated traditional Chinese and western medicine,in order to provide reference for clinical treatment of nonambulatory paraplegia in cats.
