摘要 目的比较宫外生长受限(EUGR)早产儿与适于胎龄早产儿氨基酸代谢的差异,并研究EUGR早产儿的氨基酸代谢特点。方法选取2020年4月至2021年10月佛山市妇幼保健院新生儿科收治的胎龄<32周的103例早产儿作为研究对象,根据出院时或纠正胎龄36周时(以先达到者为准)对早产儿的体重进行评估,体重低于同胎龄儿第10百分位者判定为EUGR,将研究对象分为EUGR组(n=41)及对照组(n=62)。所有研究对象在出院或纠正胎龄36周(以先达到者为准)且已停止静脉使用氨基酸>1周时采足跟血,测定研究对象20种氨基酸水平。结果EUGR组苏氨酸、精氨酸、瓜氨酸水平高于对照组,差异有统计学意义(P<0.05);EUGR组甘氨酸、谷氨酸、亮氨酸、缬氨酸水平低于对照组,差异有统计学意义(P<0.05)。结论EUGR早产儿的氨基酸代谢水平与对照组存在差异,临床医生应根据EUGR早产儿的氨基酸代谢特点提供合理营养,本研究为EUGR早产儿合理补充营养物质、及时纠正代谢紊乱和监测营养代谢变化提供重要的参考依据。 Objective To compare the differences of amino acid metabolism between preterm infants with extrauterine growth restriction(EUGR)and those appropriate for gestational age,and to study the characteristics of amino acid metabolism in preterm infants with EUGR.Methods A total of 103 preterm infants with gestational age<32 weeks admitted to Department of Neonatology of Foshan Women and Children Hospital from April 2020 to October 2021 were selected as research objects.According to the weight of children at discharge or 36 weeks of corrected gestational age(whichever comes first),those whose weight were lower than the 10th percentile of children of the same gestational age were determined as EUGR,and the subjects were divided into EUGR group(n=41)and control group(n=62).When all subjects were discharged or corrected for 36 weeks of gestational age(whichever comes first)and had stopped using amino acids intravenously for more than 1 week,heel blood droplets were collected.The levels of 20 amino acids were measured by non-derivative method.Results A total of 103 preterm infants with gestational age<32 weeks were included,including 41 in the EUGR group and 62 in the control group.Compared with the control group,the levels of threonine,arginine and citrulline in the EUGR group were higher than those in the control group,the differences were statistically significant(P<0.05).The levels of glycine,glutamate,leucine and valine in the EUGR group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion There were differences in amino acid metabolism between EUGR preterm infants and the control group.Clinicians should provide reasonable nutrition according to the amino acid metabolic characteristics of EUGR preterm infants,and this study provides an important reference for reasonable nutrient supplementation,timely correction of metabolic disorders and monitoring of nutritional metabolic changes in EUGR preterm infants.
