摘要
目的探讨联合抗核小体(Nuc)、抗双链DNA(dsDNA)抗体及抗核内核糖蛋白/Sm(nRNP/Sm)抗体定量检测对系统性红斑狼疮(SLE)的早期诊断价值。方法回顾性选取2020年6月至2021年6月景德镇市第二人民医院收治的138例自身免疫疾病患者为研究对象,其中77例SLE患者为SLE组,61例其他自身免疫疾病患者为非SLE组,另选取同期70名健康体检者为对照组。三组研究对象均进行抗Nuc、抗dsDNA、抗nRNP/Sm抗体检测,分析三者及联合检测对SLE的诊断效能。结果SLE组的抗Nuc、抗dsDNA、抗nRNP/Sm抗体水平高于非SLE组、对照组,差异有统计学意义(P<0.05);非SLE组的抗Nuc抗体水平高于对照组,差异有统计学意义(P<0.05)。活动组抗dsDNA阳性表达率高于非活动组,差异有统计学意义(P<0.05)。两组的抗Nuc抗体、抗nRNP/Sm抗体阳性表达率比较,差异无统计学意义(P>0.05)。抗Nuc、抗dsDNA抗体、抗nRNP/Sm抗体联合检测的灵敏度为93.50%、特异度为81.43%、AUC值为0.986。结论联合抗Nuc、抗dsDNA、抗nRNP/Sm抗体定量检测在SLE诊断中具有较好的补充作用,能够有效提高敏感度从而避免漏检,这对于SLE的早期诊断有较高的临床应用价值。
Objective To explore the value of combined quantitative detection of anti nucleosome(Nuc),anti double stranded DNA(dsDNA)antibodies and anti nuclear ribonucleoprotein/sm(nRNP/Sm)antibodies in the early diagnosis of systemic lupus erythematosus(SLE).Methods A total of 138 patients with autoimmune diseases admitted to the Second People′s Hospital of Jingdezhen from June 2020 to June 2021 were retrospectively selected as the research objects.Among them,77 patients with SLE were selected as SLE group,61 patients with other autoimmune diseases were selected as non-SLE group,and 70 healthy people were selected as control group.Anti-Nuc,anti-dsDNA and anti-nRNP/Sm antibodies were detected in all the three groups,and the diagnostic efficacy of the three and their combined detection for SLE was analyzed.Results The levels of anti-Nuc,anti-dsDNA and anti-nRNP/Sm antibodies in SLE group were higher than those in non-SLE group and control group,and the differences were statistically significant(P<0.05).The level of anti-Nuc antibody in non-SLE group was higher than that in control group,and the difference was statistically significant(P<0.05).The positive expression rate of anti-dsDNA in active group was higher than that in inactive group,and the difference was statistically significant(P<0.05).There was no significant difference in the positive expression rates of anti-Nuc antibody and anti-nRNP/Sm antibody between the two groups(P>0.05).The sensitivity,specificity and AUC of the combined detection of anti-Nuc,anti-dsDNA and anti-nRNP/Sm antibodies were 93.50%,81.43%and 0.986,respectively.Conclusion combined quantitative detection of anti NUC,anti dsDNA and anti nRNP/sm antibodies has a good complementary role in the diagnosis of SLE,which can effectively improve the sensitivity and avoid missed detection,which has a good clinical application value for the early diagnosis of SLE.
