摘要
目的 观察自拟清热利湿通络方联合依托考昔片治疗急性痛风性关节炎的效果及对血清炎性因子的影响。方法 选取2020年5月—2022年5月武汉大学恩施临床学院中医部收治的急性痛风性关节炎患者96例,依据随机数字表法分为对照组与研究组,各48例。2组均给予常规治疗,对照组在常规治疗基础上予以依托考昔片,研究组在对照组治疗的基础上另给予自拟清热利湿通络方,2组均持续治疗1周。比较2组临床疗效及治疗前后临床症状(关节肿胀、关节疼痛、关节功能)评分、生化指标[红细胞沉降率(ESR)、血尿酸(BUA)]、血清炎性因子[肿瘤坏死因子-α(TNF-α)、白介素(IL)-6、IL-1β、IL-8、IL-10],观察记录治疗期间的不良反应。结果 治疗1周后,研究组总有效率高于对照组(95.83%vs.81.25%,χ~2/P=5.031/0.025);治疗1周后,2组关节肿胀、关节疼痛、关节功能评分、ESR、BUA均降低,且研究组显著低于对照组(t/P=2.929/0.004、2.591/0.011、2.436/0.017、14.394/<0.001、11.210/<0.001);治疗1周后,2组血清TNF-α、IL-6、IL-1β、IL-8、IL-10水平均降低,且研究组低于对照组(t/P=9.758/<0.001、12.054/<0.001、8.203/<0.001、10.532/<0.001、10.833/<0.001);治疗期间2组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论 清热利湿通络方联合依托考昔片治疗能减轻急性痛风性关节炎患者的相关症状,降低血清炎性因子水平,疗效显著,且安全可靠。
Objective To observe the effect of Self-made Qingre Lishi Tongluo Formula combined with etocoxib in treating acute gouty arthritis and its influence on serum inflammatory factors.Methods From May 2020 to May 2022,96 patients with acute gouty arthritis admitted by the Department of Traditional Chinese Medicine of Enshi Clinical College of Wuhan University were selected and divided into the control group and the research group according to the random number table,with 48 patients in each group.Both groups were given conventional treatment,the control group was given corxi tablets on the basis of conventional treatment,and the research group was given self prepared heat clearing,dampness removing and collaterals dredging formula on the basis of the control group.Both groups were treated for 1 week.Compare the clinical efficacy of the two groups and the scores of clinical symptoms(joint swelling,joint pain,joint function),biochemical indicators [erythrocyte sedimentation rate(ESR),blood uric acid(BUA)],and serum inflammatory factors(tumor necrosis factor) before and after treatment tumor necrosis factor-α(TNF-α),Interleukin(IL)-6,IL-1 β,IL-8,IL-10,observe and record adverse reactions during treatment.Results After one week of treatment,the total effective rate of the study group was higher than that of the control group(95.83% vs.81.25%,χ~2/P=5.031/0.025).After one week of treatment,joint swelling,joint pain,joint function score,ESR,BUA in both groups decreased,and the study group was lower than the control group(t/P=2.929/0.004,2.591/0.011,2.436/0.017,14.394/0.05).Conclusion The combination of Self-made Qingre Lishi Tongluo Formula and Kaoxi Tablet can reduce the related symptoms of patients with acute gouty arthritis,reduce the level of serum inflammatory factors,with significant efficacy and safety.
