摘要
目的探究支气管哮喘病儿血清分泌型卷曲相关蛋白5(SFRP5)、CXC趋化因子受体4(CXCR4)、8-异构前列腺素(8-iso PG)水平与气道炎症、气道重塑的关系及联合评估病情程度的价值。方法选取2018年1月至2020年8月攀枝花市中心医院收治的支气管哮喘病儿113例作为研究对象,按照病情程度分为轻度组、中度组、重度组,比较三组一般资料、血清SFRP5、CXCR4、8-iso PG水平、气道炎症指标[肿瘤坏死因子-α(TNF-α)、可溶性人基质裂解素2(sST2)、白细胞介素-17(IL-17)]、气道重塑指标[痰液转化生长因子β1(TGF-β1)、基质金属蛋白酶9(MMP-9)、骨桥蛋白(OPN)],分析血清SFRP5、CXCR4、8-iso PG与气道炎症、气道重塑指标及病情程度的关系,评价血清SFRP5、CXCR4、8-iso PG对病情程度的评估价值。结果ACT评分、FEV_(1)占预估值、PEF占预估值、FEV_(1)/FVC:重度组<中度组<轻度组(P<0.05);重度组血清CXCR4、8-iso PG水平分别为(10.69±3.56)μg/L、(37.58±12.14)ng/L,高于中度组[(5.34±1.77)μg/L、(24.19±8.06)ng/L]和轻度组[(2.16±0.72)μg/L、(16.32±5.40)ng/L],且中度组血清CXCR4、8-iso PG高于轻度组(P<0.05);重度组血清SFRP5为(6.05±1.93)μg/L,低于中度组[(8.56±2.05)μg/L]、轻度组[(10.63±2.11)μg/L],且中度组血清SFRP5低于轻度组(P<0.05);血清TNF-α、sST2、IL-17、TGF-β1、MMP-9、OPN:重度组>中度组>轻度组(P<0.05);在调整ACT评分、FEV_(1)占预估值、PEF占预估值、FEV_(1)/FVC、气道炎症、气道重塑指标等其他因素后,血清SFRP5、CXCR4、8-iso PG仍与支气管哮喘病儿病情程度显著关联(P<0.05);ROC分析显示:血清SFRP5、CXCR4、8-iso PG及其联合应用对支气管哮喘病儿病情程度的评估价值,AUC(95%CI)分别为0.72(0.51,0.94)、0.81(0.69,0.93)、0.75(0.51,0.99)、0.87(0.83,0.92)。结论血清SFRP5、CXCR4、8-iso PG可通过调控支气管哮喘气道炎症、气道重塑,影响病情程度,联合检测三者水平有望为病情程度的评�
Objective To explore the relationship between serum secreted frizzled-related protein 5(SFRP5),CXC chemokine re-ceptor 4(CXCR4),8-isoprostane(8-iso PG)levels and airway inflammation and airway remodeling in children with bronchial asthma,and the value of joint assessment of disease severity.Methods A total of 113 children with bronchial asthma from January 2018 to Au-gust 2020 in Panzhihua Central Hospital were selected as the research objects,and they were assigned into mild group,moderate group,and severe group according to the severity of the disease.The general data,serum SFRP5,CXCR4,8-iso PG levels,airway inflamma-tion indexes[tumor necrosis factor-α(TNF-α),soluble human stromal lysin 2(sST2),interleukin-17(IL-17)],airway remodeling indica-tors[sputum transforming growth factorβ1(TGF-β1),matrix metalloproteinase 9(MMP-9),and osteopontin(OPN)]of the 3 groups were compared.The relationship between serum SFRP5,CXCR4,8-iso PG and airway inflammation,airway remodeling indicators and dis-ease severity was analyzed,and the evaluation value of serum SFRP5,CXCR4,8-iso PG on the disease severity was evaluated.Re⁃sults The scores of act,FEV_(1),PEF,FEV_(1)/FVC:severe group<moderate group<mild group(P<0.05).The serum levels of CXCR4 and 8-iso PG in severe group were(10.69±3.56)μg/L,(37.58±12.14)ng/L,which were higher than those in the moderate group[(5.34±1.77)μg/L,(24.19±8.06)ng/L]and mild group[(2.16±0.72)μg/L,(16.32±5.40)ng/L],and the levels of CXCR4 and 8-iso PG in moderate group were higher than those in mild group(P<0.05).Sfrp5 in severe group was(6.05±1.93)μg/L,which was lower than that in the mod-erate group[(8.56±2.05)μg/L],mild group[(10.63±2.11)μg/L],and serum Sfrp5 in moderate group was lower than that in mild group(P<0.05).Serum TNF-α,sST2,IL-17,TGF-β1,MMP-9,OPN:severe group>moderate group>mild group(P<0.05).After adjusting the act score,FEV_(1),PEF,FEV_(1)/FVC,airway inflammation,airway remodeling and other factors,serum Sfrp5,CXCR4,8-iso PG were still significantly associated with t
