^(18)F-FDG PET/CT参数与肺癌PD-L1表达的相关性研究 被引量:3

摘要 目的分析肺癌患者^(18)F-氟代脱氧葡萄糖正电子发射断层显像/计算机断层扫描(^(18)F-Fludeoxyglucose Positron Emission Computed Tomography/Computed Tomography,^(18)F-FDG PET/CT)参数最大标准摄取(Standardized Uptake Value,SUV_(max))值、肿瘤代谢体积(Metabolic Tumor Volume,MTV)、病灶糖酵解总量(Total Lesion Glycolysis,TLG)与癌组织程序性死亡蛋白配体1(Programmed Cell Death-1 Ligand 1,PD-L1)表达的相关性。方法回顾性分析2018年6月至2021年5月我院收治的90例肺癌患者(研究组)及82例肺部良性疾病患者(对照组)的临床资料,均行^(18)F-FDG PET/CT检查,并检测研究组肿瘤切除术后的癌组织、癌旁组织、对照组良性肿瘤组织中的PD-L1表达量。对比2组^(18)F-FDG PET/CT参数、PD-L1表达量,并分析^(18)F-FDG PET/CT参数与癌组织PD-L1表达的相关性。结果研究组^(18)F-FDG PET/CT参数SUV_(max)值、MTV、TLG均大于对照组,肺鳞癌SUV_(max)值、MTV、TLG均大于肺腺癌,癌组织PD-L1表达高于癌旁组织、良性肿瘤组织,差异均有统计学意义(P<0.05),良性肿瘤组织PD-L1表达高于癌旁组织(t=12.098,P<0.001);肺鳞癌癌组织PD-L1表达低于肺腺癌,差异均有统计学意义(P<0.05);研究组SUV_(max)值、MTV、TLG与癌组织PD-L1表达呈正相关(P<0.05),肺鳞癌、肺腺癌患者中结果与上述一致(肺鳞癌:r=0.769、0.878、0.905,P=0.012、0.005、<0.001;肺腺癌:r=0.744、0.815、0.846,P=0.030、0.019、0.010)。结论肺癌患者^(18)F-FDG PET/CT参数SUV_(max)值、MTV、TLG与癌组织PD-L1表达均高于癌旁组织及良性肿瘤组织,且^(18)F-FDG PET/CT参数与癌组织PD-L1表达呈正相关。 Objective To analyze the correlation between the standardized uptake value(SUV_(max)),metabolic tumor volume(MTV),total lesion glycolysis(TLG)of ^(18)F-fludeoxyglucose positron emission computed tomography/computed tomography(^(18)F-FDG PET/CT)semiquantitative parameters and the expressions of programmed cell death-1 ligand 1(PD-L1)in cancer tissues.Methods The clinical data of 90 patients with lung cancer(study group)and 82 patients with benign lung disease(control group)admitted to our hospital from June 2018 to May 2021 were analyzed retrospectively.^(18)F-FDG PET/CT examination was performed in both groups,and the expressions of PD-L1 in the cancer tissues and adjacent tissues of the study group,and benign tumor tissues of the control group after tumor resection were detected.The SUV_(max) value,MTV and TLG of ^(18)F-FDG PET/CT quantitative parameter as well as the expressions of PD-L1 in the cancer tissues,in the two groups were compared.The correlations between SUV_(max) value,MTV,TLG and the expression of PD-L1 in cancer tissues were analyzed.Results The SUV_(max) value,MTV and TLG of ^(18)F-FDG PET/CT quantitative parameter in the study group were higher than those of the control group,the SUV_(max) value,MTV and TLG of lung squamous cell carcinoma were higher than those of lung adenocarcinoma,the expression of PD-L1 in the cancer tissues of the study group was higher than those in the adjacent tissues and benign tumor tissues,the difference was statistically significant(P<0.05);and the expression of PD-L1 in benign tumor tissues was higher than that in adjacent tissues(t=12.098,P<0.001);the expression of PD-L1 in lung squamous cell carcinoma was lower than that in lung adenocarcinoma,the difference was statistically significant(P<0.05);the SUV_(max) value,MTV and TLG of ^(18)F-FDG PET/CT quantitative parameter in the study group were positively correlated with the expression of PD-L1 in cancer tissues(P<0.05),and the results of lung squamous cell carcinoma and lung adenocarcinoma were consistent with the
