摘要
目的探讨甲状腺癌患者应用甲状腺全切术治疗的临床意义,寻找最佳治疗方式。方法于本院诊治的甲状腺癌患者中遴选128例为探究对象,以随机综合平衡法分成研究组、对照组,分别纳入64例,研究起止日期为2019年6月-2021年6月。治疗过程中对照组应用甲状腺次全切术治疗,研究组应用甲状腺全切术治疗,对比各组患者临床疗效、相关指标、甲状腺功能指标、并发症发生率与复发率。结果研究组临床总有效率为93.75%,与对照组(89.06%)无差异,P>0.05;研究组并发症发生率为15.63%,显著较对照组(4.69%)高,P<0.05;研究组复发率为3.13%,显著较对照组(12.50%)低,P<0.05;研究组手术时间、住院时间较对照组短,术中出血量较对照组少,P<0.05;研究组治疗后PTH、TG、TSH水平与对照组间均无统计学差异,P>0.05。结论予以甲状腺癌患者甲状腺全切术治疗效果与次全切术治疗效果相当,且两种手术均可改善甲状腺功能,但是甲状腺全切术可缩短手术及住院时间,减少出血量,同时还可降低复发率,应用价值理想。
Objective To explore the clinical significance of total thyroidectomy in the treatment of patients with thyroid cancer,and to find the best treatment method.Methods A total of 128 cases of thyroid cancer patients diagnosed and treated in our hospital were selected as the research objects,and divided into a study group and a control group by random comprehensive balance method,and 64 cases were included respectively.The start and end dates of the study were from June 2019 to June 2021.During the treatment,the control group was treated with subtotal thyroidectomy,and the study group was treated with total thyroidectomy.Results The total clinical effective rate of the study group was 93.75%,which was no different from that of the control group(89.06%),P>0.05;the incidence of complications in the study group was 15.63%,which was significantly higher than that of the control group(4.69%),P<0.05;The recurrence rate in the group was 3.13%,which was significantly lower than that in the control group(12.50%),P<0.05;the operation time and hospital stay in the study group were shorter than those in the control group,and the intraoperative blood loss was less than that in the control group,P<0.05;There was no significant difference in the levels of PTH,TG and TSH between the control group and the control group,P>0.05.Conclusion The therapeutic effect of total thyroidectomy is comparable to that of subtotal thyroidectomy in patients with thyroid cancer,and both operations can improve thyroid function,but total thyroidectomy can shorten the operation and hospitalization time,reduce the amount of bleeding,and also Reduce recurrence rate,ideal application value.
