摘要
目的:了解慢性乙型肝炎合并糖尿病病人居家自我管理现状。方法:采用质性研究中的现象学研究方法,选取2020年7月-2020年10月武汉市某三级甲等医院住院的慢性乙型肝炎合并糖尿病病人12例进行半结构式访谈,并采用Colaizzi 7步分析法对访谈资料进行转录、编码、归纳、分析和提炼。结果:共提炼出4个主题、7个亚主题。4个主题为自我管理知识缺乏、自我管理信念不坚定、自我管理行为依从性不足、自我管理压力较大。自我管理知识缺乏包括疾病知识缺乏、疾病信息渠道缺乏2个亚主题;自我管理信念不坚定包括积极面对、消极应答2个亚主题;自我管理行为依从性不足包括就医延迟、服药依从性差、饮食管理欠科学3个亚主题。结论:慢性乙型肝炎合并糖尿病病人居家自我管理知识缺乏,自我管理负担较重。护理人员应加强自身业务能力,强化对此类病人的健康教育及延续性护理干预,及时给予有针对性的指导和帮助,以提高病人对疾病的认知程度和自我管理能力,减轻病人的疾病管理压力,从而降低病人的再入院率。
Objective:To understand the status quo of home self-management of patients with chronic hepatitis B complicated with diabetes mellitus.Methods:From July 2020 to October 2020,a total of 12 patients with chronic hepatitis B combined with diabetes mellitus who were hospitalized in a third Grade A hospital in Wuhan were selected for semi-structured interviews using phenomenological research methods in qualitative research. Colaizzi′s 7-step analysis method was used to transcribe, code, summarize, analyze and refine the interview data.Results:A total of 4 themes and 7 subthemes were extracted.The four themes were lack of self-management knowledge, unfirm self-management belief, lack of self-management behavior compliance, and high self-management pressure.Lack of self-management knowledge included two subthemes: lack of disease knowledge and lack of disease information channels.The themes of unfirm self-management belief included two sub-themes: positive face and negative response.The lack of self-management behavior compliance included three subthemes: delay in seeking medical treatment, poor medication compliance, and unscientific diet management.Conclusion:Patients with chronic hepatitis B combined with diabetes mellitus are lack of self-management knowledge, and their self-management burden is heavy.Nursing staff should strengthen their own professional ability, strengthen health education and continuous nursing intervention for such patients, timely give targeted guidance and help, in order to improve patients′ awareness of the disease and self-management ability, reduce the pressure of disease management, so as to reduce the readmission rate of patients.
