摘要
目的 基于《国际功能、残疾与健康分类(儿童和青少年版)》(ICF-CY)构建脑瘫儿童功能变量图模型,分析脑瘫儿童的功能组织特征及功能变量在关系结构中的地位。方法 选取101例脑瘫儿童便利样本,采用访谈法评估ICF-CY脑瘫核心分类组合综合版。分析ICF-CY条目间的风险相关性,建立功能图谱。结果 主组元的条目共有119项条目和528条连线,包括b条目30项、d条目52项、s条目7项和e条目30项。主岛屿含有119项条目和215条连线。两个年龄组(<6岁和6~18岁)的图模型交集共有78项条目,其中含有3项及以上节点的子网络共有7个,包括认知功能(22条目)、呼吸语言(7条目)、运动(7条目)、环境设施(6条目)、环境人员(5条目)、教育条件(4条目)、骨关节与口腔的结构(3条目)。主岛屿网络结构的加权点度与接近度和拉普拉斯度均为轻度负相关。占据强连线关系的条目拥有较少的相互连通结构。结论 基于ICF-CY条目风险相关关系的脑瘫儿童功能图谱在关系强度和关系结构上均符合已有的临床知识。采用网络分析技术,可以从功能图谱中提取脑瘫康复治疗的备选靶条目,从而为发展新型康复业务范式提供依据。
Objective To construct a functional variable graph model for children with cerebral palsy based on the International Classification of Functioning,Disability and Health for Children and Youth(ICF-CY),and analyze the functional organizational characteristics of children with cerebral palsy and the status of functional variables in the relational structure.Methods A convenience sample of 101 children with cerebral palsy was selected.The comprehensive set of ICF-CY cerebral palsy core classification combinations was evaluated by interview.Results The main component had 119 items and 528 lines,including 30 items of b,52 items of d,7 items of s and 30 items of E.The main island contained 119 items and 215 links.There were 78 items in the intersection of the graph models of the two age groups(<6 years old and 6-18 years old),among which there were 7 sub-networks with 3 or more nodes,including cognitive function(22 items),breathing language(7 items),movement(7 items),environmental facilities(6 items),environmental personnel(5 items),educational conditions(4 items),and bone joint and oral structure(3 items).In the main island,the weighted degree was significantly correlated with closeness centrality and laplacian centrality,and both correlation coefficients were slightly negative.Items occupying strong connection relationships had fewer interconnected structures.Conclusions The functional graph of children with cerebral palsy based on the risk correlation of ICF-CY items conforms to the existing clinical knowledge in terms of relationship strength and structure.The network analysis technique can extract a list of candidate target items for therapeutic usages and then support to the development of a new paradigm for rehabilitation practices.
