摘要
目的探讨结直肠内镜下黏膜剥离术(endoscopic submucosal dissection,ESD)后发生迟发性出血的影响因素。方法选择医院2019年7月—2021年4月收治的90例结直肠ESD患者为本次调查的研究对象,根据术后便血时间是否≥6 h分为迟发性出血组(n=44)和无迟发性出血组(n=46),收集息肉大小、息肉部位、息肉病理、出血标准等临床指标纳入单因素分析,根据单因素分析结果进行Logistic多因素分析,得出影响结直肠ESD术后迟发性出血的因素并制订护理对策。结果单因素分析显示,高血压病、术中出血、息肉大小、年龄、血小板为影响结直肠ESD术后发生迟发性出血的相关因素(P<0.05)。多因素Logistic回归分析显示,高血压病、术中出血、息肉大小是结直肠ESD术后发生迟发性出血的影响因素(P<0.05)。结论结直肠ESD术后患者迟发性出血的危险因素较多,其中高血压病、术中出血、息肉大小是ESD术后发生迟发性出血的主要影响因素,应对此展开科学、合理的干预措施,有效改善患者迟发性出血状况,促进预后。
Objective To explore the influencing factors of delayed bleeding after colorectal endoscopic submucosal dissection(ESD).Methods To select 90 patients with colorectal ESD admitted to the hospital from July 2019 to April 2021 as the study objects of this investigation.According to whether the blood in the stool after operation was≥6 hours,they were divided into delayed bleeding group(n=44)and non-delayed bleeding group(n=46).Clinical indicators such as polyp size,polyp location,polyp pathology,and bleeding criteria were collected and included in univariate analysis.Logistic multivariate analysis was performed according to the results of univariate analysis,and the factors af fecting delayed bleeding after colorectal ESD were obtained and nursing countermeasures were formulated.Results Univariate analysis showed that the hypertension,intraoperative bleeding,polyp size,age and platelets were the related factors affecting delayed bleeding after colorectal ESD(P<0.05).Multivariate Logistic regression analysis showed that the hypertension,intraoperative bleeding and polyp size were the factors af fecting delayed bleeding after colorectal ESD(P<0.05).Conclusion There were many risk factors for delayed bleeding in patients after colorectal ESD,among which hypertension,intraoperative bleeding and polyp size were the main influencing factors for delayed bleeding after ESD.Scientifi c and reasonable intervention measures should be carried out to ef fectively improve the delayed bleeding status of patients and promote the prognosis.
