BPH患者尿道周围前列腺组织弹性模量与临床症状相关性分析 被引量:1

Correlation between elastic modulus of periurethral prostate tissue and clinical symptoms in patients with benign prostatic hyperplasia

摘要 目的:分析前列腺增生(benign prostatehy perplasia, BPH)患者尿道周围前列腺组织剪切波弹性(shear wave elastography, SWE)与国际前列腺症状评分(international prostate symptom score, IPSS)的相关性。方法:选取2021年1月至2021年10月广州市番禺区中心医院就诊的BPH患者80例,再选取无前列腺症状的健康男性志愿者20例为正常组,对80例BPH患者根据其临床症状进行IPSS评分,将患者的评分由低至高分为轻、中、重度三组,接着对所有BPH患者及志愿者进行经直肠超声检查(transrectal ultrasonography, TRUS),通过SWE测量尿道周围前列腺组织弹性模量,同时测量BPH患者前列腺径线计算出前列腺重量,将前列腺重量由低至高分为甲、乙、丙、丁四组。比较各组BPH患者及正常组之间弹性模量的差异,分析弹性模量与IPSS、前列腺重量以及患者年龄的相关性。结果:BPH患者不同区域尿道周围前列腺组织弹性模量均随着IPSS、前列腺重量以及患者年龄的增加而升高,差异均具有统计学意义(P<0.05);通过相关性分析发现,BPH患者各个区域尿道周围前列腺组织弹性模量与IPSS均呈正相关,上部A区(r=0.739,P<0.01)、上部B区(r=0.771,P<0.01)、中部A区(r=0.751,P<0.01)、中部B区(r=0.758,P<0.01)、下部A区(r=0.725,P<0.01)、下部B区(r=0.762,P<0.01);以是否存在前列腺症状为标准,绘制前列腺周围尿道组织A区及B区剪切波弹性模量诊断BPH的ROC曲线,其线下面积分别为0.958及0.966。结论:BPH患者尿道周围前列腺组织弹性模量与临床症状呈正相关,因此可把SWE作为评估前列腺增生严重程度的方法。 Objective:To analyze the correlation between shear wave elasticity(SWE) of periurethral prostate tissue and international prostate symptom score(IPSS) in patients with benign prostatic hyperplasia(BPH).Methods:80 patients with BPH treated in Guangzhou Panyu Central Hospital from January 2021 to October 2021 were selected as the research object, and 20 healthy male volunteers without prostate symptoms were selected as the normal group. According to the clinical symptoms, 80 patients with BPH were scored by IPSS. The scores of patients were divided into mild group, moderate group and severe group from low to high.Then transrectal ultrasonography(TRUS) was performed in all BPH patients and volunteers, the elastic modulus of prostate tissue around the urethra was measured by SWE, and the prostate size diameter of BPH patients was measured to calculate the prostate weight. The prostate weight of patients was divided into group A, group B, group C and group D from low to high. The differences of elastic modulus between BPH patients and normal group were compared, and the correlation between elastic modulus and IPSS, prostate weight and patient age was analyzed.Results:The elastic modulus of periurethral prostate tissue in different regions of BPH patients increased with the increase of IPSS, prostate weight and patients’ age, and the difference was statistically significant(P<0.05);Through correlation analysis, it was found that the elastic modulus of periurethral prostate tissue in BPH patients was positively correlated with IPSS,upper region area A(r=0.739, P<0.01),upper area B(r=0.771, P<0.01),middle area A(r=0.751, P<0.01),middle area B(r=0.758, P<0.01), lower area A(r=0.725, P<0.01) and lower area B(r=0.762, P<0.01), taking the presence of prostate symptoms as the standard, the ROC curves of shear wave elastic modulus in area a and area B of urethral tissue around the prostate for the diagnosis of BPH were drawn, and the areas under the line were 0.958 and 0.966.Conclusion:The elastic modulus of periurethral pro
