摘要
目的观察地佐辛复合舒芬太尼用于老年人腹部手术术后镇痛的临床疗效。方法80例行腹部手术的老年患者,采用随机分配的方式分为对照组和观察组,每组40例。对照组患者术后应用舒芬太尼进行镇痛,观察组患者术后应用地佐辛复合舒芬太尼进行镇痛。比较两组患者不良反应发生情况及术后不同时间点视觉模拟评分法评分、Ramsay镇静评分。结果两组患者术后2、6、12、24、36、48 h的视觉模拟评分法评分比较,差异无统计学意义(P>0.05)。观察组患者术后2、6、12、24、36、48 h的Ramsay镇静评分分别为(2.51±0.85)、(2.45±0.52)、(2.33±0.55)、(2.43±0.66)、(2.36±0.55)、(2.43±0.23)分,明显优于对照组的(1.91±0.80)、(1.91±0.32)、(1.90±0.31)、(1.91±0.50)、(1.83±0.33)、(1.91±0.56)分,差异具有统计学意义(P<0.05)。观察组不良反应发生率为5.0%,低于对照组的37.5%,差异具有统计学意义(P<0.05)。结论针对老年腹部手术患者术后采用地佐辛复合舒芬太尼进行镇痛,可以获取更加良好的镇痛效果,同时能够降低各种不良反应的发生率,具有一定的临床推广价值。
Objective To observe the clinical efficacy of dezocine combined with sufentanil for postoperative analgesia in elderly patients undergoing abdominal surgery.Methods A total of 80 elderly patients who underwent abdominal surgery were randomly assigned to control group and observation group,with 40 cases in each group.The control group received sufentanil for postoperative analgesia,and the observation group received dezocine combined with sufentanil for postoperative analgesia.The incidence of adverse reactions,visual analogue scale score and Ramsay sedation score at different time points after surgery were compared between the two groups.Results There was no statistically significant difference in the visual analogue scale scores between the two groups at 2,6,12,24,36,and 48 h after surgery(P>0.05).The Ramsay sedation scores in the observation group at 2,6,12,24,36,and 48 h after surgery were(2.51±0.85),(2.45±0.52),(2.33±0.55),(2.43±0.66),(2.36±0.55),and(2.43±0.23)points,which were significantly better than(1.91±0.80),(1.91±0.32),(1.90±0.31),(1.91±0.50),(1.83±0.33),and(1.91±0.56)points in the control group,and the differences were statistically significant(P<0.05).The incidence of adverse reactions in the observation group was 5.0%,which was lower than 37.5%in the control group,and the difference was statistically significant(P<0.05).Conclusion Dezocine combined with sufentanil for postoperative analgesica in elderly abdominal surgery patients can obtain better analgesic effect and reduce the incidence of various adverse effects,which has certain clinical promotion value.
