摘要
目的探讨关节镜下外侧支持带松解术(LRR)联合髌骨内侧韧带重建(MPFL-R)术在青少年复发性髌骨脱位(RDP)中的治疗效果。方法回顾性分析采用开放性内侧支持带紧缩治疗的56例青少年RDP患者(A组)和采用关节镜下LRR联合MPFL-R术治疗的78例青少年RDP患者(B组)的临床资料。对比手术指标及并发症。结果B组术中出血量少于A组(P<0.05),手术时间、住院时间均短于A组(P<0.05);术后12个月两组适合角、外侧髌股角、Lysholm评分、Kujala评分均大于治疗前(P<0.05),且B组均大于A组(P<0.05);B组并发症总发生率低于A组(P<0.05)。结论关节镜下LRR联合MPFL-R术治疗青少年复发性髌骨脱位效果理想,并发症少。
Objective To explore the effect of arthroscopic lateral retinaculum release(LRR)combined with medial patellofemoral ligament-reconstruction(MPFL-R)in the treatment of juvenile recurrent dislocation of patellar(RDP).Methods The clinical data of 56 adolescent RDP patients treated with open medial retinaculum constriction(group A)and 78 adolescent RDP patients treated with arthroscopic LRR combined with MPFL-R(group B)were analyzed retrospectively.The operative indexes and complications were compared.Results There were fewer intraoperative blood loss and shorter operation time and length of stay in group B(P<0.05)(P<0.05).The suitable angles and lateral patellofemoral angles,Lysholm scores and Kujala scores in the two groups were higher than those before treatment(P<0.05),particularly in group B(P<0.05).The total incidence of complications in group B was lower than that in group A(P<0.05).Conclusion Arthroscopic LRR combined with MPFL-R for the treatment of RDP in adolescents has good effect and less complications.
