摘要
【目的】探讨透骨血竭散外敷对类风湿性关节炎(RA)大鼠的治疗作用及机制。【方法】将44只大鼠随机分为假手术组、模型组、透骨血竭散组及扶他林组,每组11只。除假手术组外,其余各组大鼠构建RA模型。建模成功后,次日透骨血竭散组大鼠于左后足足跖部及踝关节外敷透骨血竭散,扶他林组相同部位外敷扶他林软膏,模型组及假手术组相同部位外敷空白凝胶膏剂。采用足容积测量仪测量大鼠左后足的肿胀体积;苏木素-伊红(HE)染色法观察踝关节病理形态并进行炎症细胞浸润评分;末端脱氧核苷酸转移酶(TdT)介导的d UTP缺口末端标记(TUNEL)法测定滑膜细胞凋亡;蛋白免疫印迹法检测踝关节组织T细胞抗原受体(TCR)通路中Lck、Fyn蛋白表达;免疫组织化学法检测踝关节滑膜组织Bax、Bcl-2阳性表达。【结果】与假手术组比较,模型组大鼠足爪肿胀度升高,踝关节组织呈现明显RA病理改变,且炎症细胞浸润评分升高,踝关节组织Lck、Fyn蛋白表达水平升高,滑膜组织Bcl-2阳性细胞比率升高、Bax阳性细胞比率降低(均P<0.05);与模型组比较,透骨血竭散组及扶他林组的足爪肿胀度降低,踝关节组织病理损伤改善,且炎症细胞浸润评分降低,踝关节组织Lck、Fyn蛋白表达水平降低,滑膜组织Bcl-2阳性细胞比率降低、Bax阳性细胞比率升高(均P<0.05);透骨血竭散组各指标的改善效果优于扶他林组(均P<0.05)。【结论】透骨血竭散外敷可改善RA大鼠踝关节的肿胀,其机制可能与抑制TCR信号通路Lck、Fyn蛋白表达,抑制炎症细胞浸润,促滑膜细胞凋亡有关。
Objective To explore the therapeutic effect and mechanism of external application of Tougu Xuejie Powder for rheumatoid arthritis(RA)rats.Methods Forty-four rats were randomly divided into the sham operation group,model group,Tougu Xuejie Powder group and Voltaren group,with 11 rats in each group.Except for the sham operation group,RA model was constructed in other rats.The next day after the successful modeling,the rats in the Tougu Xuejie Powder group were given external application of Tougu Xuejie Powder on the left rear full metatarsal and ankle joints,and the same parts of the Voltaren group were externally applied with Voltaren ointment,and the same parts of the model group and the sham operation group were externally applied with blank gel ointment.The swelling volume of left hind foot was measured by foot volume meter.The pathological morphology of ankle joint was observed by hematoxylin-eosin(HE)staining and inflammatory cell infiltration scoring was performed.Terminal deoxynucleotide transferase(TdT)mediated dUTP notch terminal labeling(TUNEL)assay was used to determine synovial cell apoptosis.The protein expression of Lck and Fyn in T cell antigen receptor(TCR)pathway was detected by Western Blot;the positive expression of Bax and Bcl-2 in synovial tissue of ankle joint was detected by immunohistochemistry.Results Compared with the sham operation group,the rat paw swelling degree of the model group was increased,obvious RA pathological changes in ankle tissue were seen,inflammatory cell infiltration score was increased,the protein expression levels of Lck and Fyn in ankle tissue were increased,the ratio of Bcl-2 positive cells in the sivium tissue was increased and the ratio of Bax positive cells was decreased(all P<0.05);compared with the model group,the paw swelling degree of the Tougu Xuejie Powder group and the Voltaren group was decreased,the pathological injury of the ankle tissue was improved,and the inflammatory cell infiltration score was decreased,the protein expression levels of Lck and Fyn
