摘要
中心静脉闭塞性疾病(central venous occlusive disease, CVOD)是维持性血透患者常见且严重的血管通路并发症之一。中心静脉透析导管留置时间、位置、管径及透析内瘘的高流量是血透相关CVOD的主要危险因素。经皮球囊扩张血管成形术(percutaneous transluminal angioplasty, PTA)和经皮支架植入血管成形术(percutaneous transluminal angioplasty with stent placement, PTS)是症状性CVOD的主要治疗手段。CVOD据不同病因、病变严重程度和解剖位置,其治疗难度与远期预后有所不同,需考虑不同的治疗策略。目前关于CVOD的临床研究局限于小样本回顾性观察研究,基础研究较匮乏,CVOD诊治新技术与新器械需在未来的研究中进一步探索和验证。
Central venous occlusive disease(CVOD) is a common and intractable complication which can directly lead to dysfunction of vascular access in patients on maintenance hemodialysis. The indwelling duration, location and diameter of central venous dialysis catheter, and dialysis fistula with high-flow are the mainly known risk factors for CVOD. Percutaneous transluminal angioplasty(PTA) and percutaneous transluminal angioplasty with stent placement(PTS) are recommended as the first-line treatment strategy for symptomatic CVOD.CVOD shows varied difficulties in treatment and different long-term outcomes because of the different etiologies, severity of the lesion and anatomical locations of the lesion, hencetargeted interventional strategies should be considered.At present, clinical studies on CVOD are limited to retrospective observational reports with small samples, and experimental studies are scarce. New techniques and instruments for the diagnosis and treatment of CVOD need to be further explored and verified in future studies.
