摘要
目的:研究超声弹性成像联合血清三叶因子1(trefoil factor 1,TFF1)、孕激素受体膜组分1(progesterone receptor membrane 1,PGRMC1)在乳腺癌早期筛查中的诊断价值。方法:选取我院于2018年6月~2020年12月收治的148例乳腺病变患者,均行超声弹性成像检查,检测患者外周血TFF1、PGRMC1水平。根据组织病理学以及影像学检测结果分为良性组、恶性组,比较两组患者超声弹性成像以及血清TFF1、PGRMC1水平差异;绘制受试者工作特征曲线(receiver operating characteristic curve,ROC曲线),评估超声弹性成像联合血清TFF1、PGRMC1检测对乳腺良恶性病变的诊断价值。结果:恶性组患者TFF1、PGRMC1水平显著高于良性组(P<0.05)。ROC曲线分析显示,TFF1诊断恶性病变的截断值为1.309 ng/mL,AUC(95%CI)为0.807(0.739~0.875),PGRMC1诊断恶性病变的截断值为44.02 ng/L,AUC(95%CI)为0.847(0.784~0.910)。恶性组患者超声弹性成像评分高于良性组(P<0.05),其中恶性病变特征多表现为边界不清晰、形状不规则、无钙化灶、内部回声不均、血流分级为Ⅱ~Ⅲ级。超声弹性成像以及外周血TFF1、PGRMC1联合诊断的敏感度(97.8%)、阴性预测率(95.8%)以及准确率(90.5%)均明显高于单独检测。结论:乳腺恶性病变者的外周血TFF1、PGRMC1处于高表达状态,且超声弹性成像特征与良性病变者明显不同,超声弹性成像联合血清TFF1、PGRMC1检测可有效早期筛查乳腺良、恶性病变。
148 patients with breast lesions in our hospital from June 2018 to December 2020 were detected by ultrasound elastography. The levels of TFF1 and PGRMC1 in peripheral blood were detected. The patients were divided into benign group and malignant group according to the histopathological and imaging results. The ultrasound elastography and the levels of serum TFF1 and PGRMC1 between the two groups were compared.Receiver operating characteristic curve(ROC)was drawn to evaluate the diagnostic value of ultrasonic elastography combined with serum TFF1 and PGRMC1 in the differential diagnosis of benign and malignant breast lesions. The levels of TFF1 and PGRMC1 in malignant group were significantly higher than those in benign group(P < 0. 05). ROC curve analysis showed that the cutoff value of TFF1 in the diagnosis of malignant lesions was 1. 309ng/mL,AUC(95% CI)was 0. 807(0. 739—0. 875),the cutoff value of PGRMC1 in the diagnosis of malignant lesions was 44. 02ng/L,AUC(95% CI)was 0. 847(0. 784—0. 910). The ultrasound elastography scores of patients in malignant group were higher than those in benign group(P<0. 05),and the malignant lesions were characterized by unclear boundary,irregular shape,no calcification,uneven internal echo,and blood flow grading of grade II-III. The sensitivity(97. 8%),negative prediction rate(95. 8%)and accuracy(90. 5%)of ultrasonic elastography combined with peripheral blood TFF1 and PGRMC1 were significantly higher than those of single detection. Ultrasonic elastography combined with serum TFF1 and PGRMC1detection can effectively screen benign and malignant breast lesions in early stage.
