摘要
目的分析对行钬激光碎石术的肾结石患者联合应用针对性护理、叙事护理的临床效果。方法以2019年1月至2021年1月在徐州医科大学附属第三医院行钬激光碎石术的86例肾结石患者为研究对象,通过随机数字表法的分组方式纳入对照组(43例)和观察组(43例)。对照组开展叙事护理,观察组基于叙事护理应用针对性护理。对比2组患者的疼痛程度、并发症发生率、康复情况及肾功能。结果护理24h、48h后及出院时,2组患者的视觉模拟评分法评分均较护理前降低,且观察组均较同期对照组更低,比较均存在统计学差异(P均<0.05)。与对照组比较,观察组的并发症总发生率更低,手术时间及住院时间均更短,双J管拔除时间和首次下床活动时间均更早,术中出血量更少,差异均有统计学意义(P均<0.05)。出院时,2组患者的肾功能水平均较护理前有所改善,且观察组的肾功能水平均较对照组更优,比较均存在统计学差异(P均<0.05)。结论联合应用叙事护理与针对性护理有助于减少行钬激光碎石术肾结石患者术后并发症的发生,在加快术后康复进程、促进肾功能水平恢复的同时缓解疼痛症状。
Objective To analyze the clinical effect of targeted nursing combined with narrative nursing on the renal calculus patients undergoing holmium laser lithotripsy.Methods A total of 86 patients with renal calculi who underwent holmium laser lithotripsy at the Third Affiliated Hospital of Xuzhou Medical University from January 2019 to January 2021 were selected as the research subjects.They were allocated into a control group(43 cases)and an observation group(43 cases)by a random number table.The control group carried out narrative nursing,and the observation group carried out targeted nursing in addition to the narrative nursing.The pain intensity,complication rate,rehabilitation,and renal function were compared between the two groups.Results After 24,48 hrs of nursing and at discharge,the visual analogue scale scores of the two groups were lower than those before nursing,and the observation group had lower scores than the control group(all P<0.05).Compared with the control group,the total complication rate in the observation group was lower.The operation time,hospitalization time,double J tube indwelling time,and first ambulation time were shorter.The intraoperative blood loss was less(all P<0.05).At discharge,the renal function levels of the two groups were improved compared with those before nursing,and the renal function levels of the observation group were better than those of the control group(all P<0.05).Conclusion Targeted nursing combined with narrative nursing can help reduce the postoperative complications,speed up the postoperative rehabilitation process,promote the recovery of renal function,and relieve the pain of renal calculus patients undergoing holmium laser lithotripsy.
