摘要
目的观察微小核糖核酸-181b(miR-181b)、E-选择素(E-selectin)、可溶性人基质裂解素2(sST2)在急性心力衰竭(AHF)中的表达及相关性。方法选取2021年1月~2022年1月于镇平县人民医院接受治疗的75例AHF患者作为AHF组研究对象,另外在同一时期选取体检健康人61名作为对照组研究对象。采用荧光定量聚合酶测定miR-181b,酶联免疫吸附法检测E-selectin、sST2。结果相比对照组,AHF组miR-181b降低,E-selectin、sST2升高(P<0.05)。相比心功能Ⅱ级,心功能Ⅲ、心功能Ⅳ级miR-181b降低,E-selectin、sST2升高(P<0.05);相比心功能Ⅲ级,心功能Ⅳ级miR-181b降低,E-selectin、sST2升高(P<0.05)。mi R-181b、E-selectin负相关(r=-0.539,P=0.001);miR-181b、sST2负相关(r=-0.439,P=0.001);E-selectin、sST2正相关(r=0.511,P=0.001)。miR-181b、E-selectin、sST2为影响AHF发生的危险因素(P<0.05)。相比miR-181b、E-selectin、sST2单一诊断,联合诊断对AHF的诊断价值较高(P<0.05)。结论AHF患者mi R-181b呈低表达,E-selectin、sST2呈高表达,且上述指标与不同心功能分级相关,是AHF发生的检测因子。
Objective To observe the expression and correlation of miR-181b,E-selectin,soluble isoform of suppression of tumor necrosis 2(sST2)in acute heart failure(AHF).Methods Seventy-fi ve AHF patients who received treatment in Zhenping county people’s Hospital from January 2021 to January 2022 were selected as the AHF group research objects,and 61 healthy people were selected as the control group research objects in the same period.miR-181b was determined by fl uorescence quantitative polymerase,and E-selectin and sST2 were detected by enzyme-linked immunosorbent assay.Results Compared with the control group,miR-181b decreased and E-selectin and sST2 increased in AHF group(P<0.05).Compared with gradeⅡof cardiac function,miR-181b decreased and E-selectin and sST2 increased in gradeⅢandⅣof cardiac function(P<0.05).Compared with gradeⅢof cardiac function,miR-181b decreased and E-selectin and sST2increased in gradeⅣof cardiac function(P<0.05).There was a negative correlation between miR-181b and E-selectin(r=-0.539、P=0.001).There was a negative correlation between miR-181b and sST2(r=-0.439、P=0.001).There was a positive correlation between E-selectin and sST2(r=-0.439、P=0.001).miR-181b,E-selectin and sST2 were the risk factors affecting the occurrence of AHF(P<0.05).Compared with the single diagnosis of miR-181b,E-selectin and sST2,the value of combined diagnosis in the diagnosis of AHF was higher(P<0.05).Conclusion In AHF patients,miR-181b is low in expression,E-selectin and sST2 are high in expression,and the above indicators are related to the non concentric functional grade,which are detection factors for the occurrence of AHF.
