摘要
目的:比较减压引流结合中医松弛挂线术与中医低位切开高位挂线术治疗括约肌上肛瘘的临床疗效。方法:60例括约肌上肛瘘患者按照随机数字表法分为对照组和观察组,每组各30例。观察组给予减压引流结合中医松弛挂线术,对照组给予中医低位切开高位挂线术。比较两组患者临床疗效、术后愈合时间、术后肛门疼痛视觉模拟量表(visual analogue scale, VAS)评分、肛门功能评分量表(Wexner)评分。结果:两组患者有效率均为100%,差异无统计学意义(P>0.05);观察组患者术后创口愈合平均时间明显低于对照组,差异有统计学意义(P<0.05);观察组患者术后1 d、术后3 d、术后1周VAS评分均显著低于同期对照组,差异均有统计学意义(P<0.05);观察组患者术后1个月、术后3个月、术后6个月Wexner评分均显著低于对照组,差异均有统计学意义(P<0.05)。结论:减压引流结合中医松弛挂线术治疗括约肌上肛瘘疗效显著,可显著缩短患者术后创口愈合时间、术后患者疼痛程度。
Objective: To compare the clinical efficacy of decompression and drainage combined with traditional Chinese medicine loose threading and traditional Chinese medicine low-position incision and high-position hanging thread therapy in the treatment of superior sphincter anal fistula.Methods: 60 patients with superior sphincter anal fistula were divided into control group and observation group according to the random number table method, with 30 cases in each group.The observation group was given decompression and drainage combined with traditional Chinese medicine loosening hanging thread therapy, while the control group was given low-position incision and high-position hanging thread therapy of traditional Chinese medicine.The clinical efficacy, postoperative healing time, postoperative anal pain visual analogue scale(VAS) score, and anal function score scale(Wexner) score were compared between the two groups.Results: The effective rate of both groups was 100%,and the difference was not statistically significant(P>0.05).The mean postoperative wound healing time of the observation group was significantly lower than that of the control group, and the difference was statistically significant(P<0.05).The VAS scores of the observation group at 1 d, 3 d, and 1 week after operation were significantly lower than those of the control group during the same period, and the differences were statistically significant(P<0.05).The Wexner scores of the observation group were significantly lower than those of the control group at 1 month, 3 months, and 6 months after the operation, and the differences were statistically significant(P<0.05).Conclusion: The combination of decompression and drainage combined with traditional Chinese medicine loosening hanging thread therapy in the treatment of superior sphincter anal fistula has a significant effect, and can significantly shorten the postoperative wound healing time and postoperative pain of patients.
