摘要
目的分析临终患者家庭的“社会-心理”困扰,为困扰解决的方法策略设计提供现实依据。方法2018年7-12月,2019年9月至2020年6月,对上海某社区医院安宁疗护科入住的4个临终患者家庭行参与式观察,2018年7月,对该科8名护士行叙事访谈,采用分析叙事法对两个视角的资料进行分析。结果临终患者家庭“社会-心理”困扰可归纳为10个类型,即:缺乏情感支持、不接受临终结局、遗愿实现受阻、担忧家庭发展、恐惧死亡过程、病情沟通不畅、不舍亲人分离、情感交流不畅、人生价值迷惘与家属身心疲乏。结论临终患者家庭“社会-心理”困扰具有普遍性、重要性、独特性、多样性、复杂性、紧迫性、反复性的特点;叙事方法可成为理解、分析、解决临终患者家庭“社会-心理”困扰的有效方法途径。
Objective To study and analyze the“social-psychological”distress of the dying patients’family and to provide practical basis for the solutions.Methods From July to December 2018,and from September 2019 to June 2020,4 dying patients’families admitted to Hospice Care Department of a Community Health Service Center in Shanghai were observed by the participatory observation method.In July 2018,narrative interviews were conducted among 8 nurses in the department.Data obtained from two research perspectives were analyzed by the narratives analysis method.Results 10 types of“social-psychological”distress were extracted,including lack of emotional support,unwillingness to accept the end-of-life outcome,unfulfillment of the last wishes,worries of the family development,fear of the process of death,troubles in disease progression communication,reluctance of separation with families,troubles in feelings sharing,lost of the values,physical and mental fatigue of the caregivers.Conclusions The“social-psychological”distress of the dying patients’family have the characteristics of universality,importance,particularity,diversity,complexity,urgency,repetition.Narrative method can be an effective way to understand,analyze and solve the distress.
