摘要
目的:探讨促性腺激素释放激素类似物(GnRHa)对特发性性早熟女童性发育及相关激素水平的影响。方法:选取我院2016年5月至2019年5月收治的90例特发性性早熟女童作为研究对象,并予以GnRHa治疗,均连续治疗12个月,并于治疗后6个月、12个月进行随访。比较患儿治疗前后身高、骨龄、预测身高变化情况与血清黄体生成素(LH)、卵泡刺激素(FSH)、雌二醇(E_(2))等性激素水平及子宫、卵巢、乳房等性发育积分情况。结果:与治疗前比较,治疗后6个月、12个月患儿的身高、预测身高变化均呈升高趋势,其中治疗后12个月高于治疗后6个月(P<0.05);治疗前、治疗后6个月、治疗后12个月骨龄比较,差异无统计学意义(P>0.05);与治疗前比较,治疗后6个月、12个月患儿的子宫、卵巢、乳房等性发育积分及血清LH、FSH、E_(2)、FSH峰值水平均下降,其中治疗后12个月低于治疗后6个月(P<0.05)。结论:GnRHa治疗特发性性早熟女童可有效缓解女童的临床症状,抑制性发育,降低相关性激素水平,控制其性早熟进展。
Objective:To investigate the effect of gonadotropin-releasing hormone analogs(Gn RHa)on the sexual development and related hormone levels in girls with idiopathic precocious puberty.Methods:A total of 90 girls with idiopathic precocious puberty admitted to our hospital from May 2016 to May 2019 were selected as the research subjects,and were treated with Gn RHa for 12 months.At 6 and 12 months after treatment,the changes of girls’height,bone age,predicted height,serum levels of LH,FSH and E_(2),and sexual development such as uterus,ovaries and breasts were compared before and after treatment.Results:Compared with before treatment,the children’s height and predicted height showed an increasing trend at 6and 12 months after treatment,and those at 12 months after treatment were higher than those at 6 months(P<0.05).There was no significant difference in skeletal age before and after treatment(P>0.05).Compared with before treatment,the scores of sexual development of girls’uterus,ovaries and breasts,and serum LH,FSH,E_(2)and FSH peak levels decreased at 6 and 12months after treatment,and these indexes at 12 months after treatment were lower than those at 6 months(P<0.05).Conclusion:Gn RHa can effectively relieve the clinical symptoms of girls with idiopathic precocious puberty,inhibit sexual development,reduce the level of related sex hormones,and control the progression of precocious puberty.
