摘要
目的 分析妇产专科医院发生医院感染患者的感染部位及病原菌分布特点,以期为妇产科患者感染的预防和治疗提供思路。方法 选取2016年1月至2021年6月首都医科大学附属北京妇产医院发生医院感染的患者资料,对其进行感染部位及病原菌分布特点的分析。结果 2016—2021年发生医院感染上报病例399例,共481例次。其中妇科238例次,平均年龄(32.5±6.6)岁,平均住院时间为(13.51±6.41)d;产科243例次,平均年龄(31.4±3.9)岁,平均住院时间为(11.78±6.34)d;妇科与产科患者的年龄与平均住院时间比较,差异均有统计学意义(P <0.05)。医院感染部位主要以生殖系统和手术创面为主,分别占43.9%和30.9%。从481例次感染标本中共分离出44种病原菌,其中革兰阴性菌检出率为67.4%,革兰阳性菌为27.9%,真菌为4.8%。除呼吸系统外其余各感染部位均以革兰阴性菌为主,其中大肠埃希菌最多。结论 妇产专科医院发生医院感染的病原菌以革兰阴性菌为主,多分布于生殖系统及手术创面,临床中应重视无菌操作并减少侵入性操作。
Objective To analyze the infection site and pathogen distribution characteristics of patients with nosocomial infection in obstetrics and gynecology hospital, and to provide ideas for the prevention and treatment of infection in obstetrics and gynecology patients. Methods Basic information of patients with nosocomial infection in Beijing Obstetrics and Gynecology Hospital, Capital Medical University from January 2016 to June 2021 was collected, the infection site and the distribution of pathogens was analyzed. Results From 2016 to 2021, a total of 481 hospital infections in 399 reported cases occurred, of which there were 238 infections in gynecology, with an average age of(32.5 ± 6.6) years and an average hospital stay of(13.51 ± 6.41) days, and there were 243 infections in obstetrics, with an average age of(31.4 ± 3.9) years and an average hospital stay of(11.78 ± 6.34) days. There were statistical differences in the age and average length of hospital stay of the patients(P < 0.05). The most common sites of nosocomial infection were the reproductive system and surgical wounds,which accounted for 43.9% and 30.9%, respectively. A total of 44 pathogenic bacteria was isolated from 481 infection specimens, of which the detection rate of Gram-negative bacteria was 67.4%, the Gram-positive bacteria was 27.9%, and the fungus was 4.8%. In addition to the respiratory system, the rest of the infection sites were dominated by Gram-negative bacteria, of which the Escherichia coli was more than others. Conclusions The pathogen are primarily Gram-negative bacteria in obstetrics and gynecology hospital, which mostly distribute in reproductive system and surgical wound. Aseptic technique should be emphasized and invasive operation should be reduced in clinical surgery.
