摘要
儿童哮喘为临床高发、病程较长、迁延难愈的疾病。发作期对因治疗,疗效尚可,但缓解期因患儿免疫功能低下、气道炎症、气道高反应性持续存在,导致的器质性损害、长期易感状态及严重并发症则很难改善,成为控制哮喘反复发作和达到长期、规范、稳定疗效的难点和重点。常规治疗往往对全身免疫功能调节的关注度不高,很难从根本上防止反复发作,增强临床控制效果。现普遍认为,运用中医中药手段治疗,可发挥治病求本特色,辨体辨证施治,着眼于改善患儿免疫低下的状态,疗效稳定。王有鹏教授从事儿科临床工作30余年,一直致力于探究哮喘行之有效的中医治疗方法。其从营卫不和的理论出发,总结该病迁延难愈,反复发作的病机关键为脏腑偏颇、营卫乏源、营失内守、卫不外固、正气虚弱、反复外感。治疗上调和营卫,使卫外滋内功能正常,减少患儿反复发作或外感的次数;健脾益气、养阴生津,调整脏腑阴阳偏颇,阴平阳秘则患儿免疫功能得以增强,形成了“调和营卫-扶助正气-改善免疫”的整体论治思路,从体质出发,针对病机,标本兼治,临床疗效较佳。
Childhood asthma is a disease with high incidence,long course and it is difficult to cure.The curative effect of cause treatment in the attack period is fair,but in the remission period,the organic damage,long-term susceptibility and serious complications caused by children's low immune function,airway inflammation and persistent airway hyperresponsiveness are difficult to treat,which have become the difficulty and focus of controlling recurrent asthma and achieving long-term,standardizing and stabilizing the curative effect.The routine treatment often pays little attention to the regulation of systemic immune function,which is difficult to fundamentally prevent recurrent attacks and enhance the clinical control effect.It is generally believed that the use of traditional Chinese medicine can give play to the characteristics of treating diseases and seeking the root,differentiating body and syndrome differentiation,focus on improving the state of children's low immunity,and the curative effect is stable.Professor WANG Youpeng has been engaged in pediatric clinical work for more than 30 years and has been committed to exploring effective traditional Chinese medicine treatment methods for asthma.Starting from the theory of disharmony between Ying-phase and Wei-phase,he summarized that the disease is difficult to heal.The key pathogenesis of repeated attacks is bias of Zang-Fu organs,lack of source of Ying-phase and Wei-phase,loss of Ying-phase and Wei-phase defense,no external fixation of Wei-phase,weakness of healthy Qi and repeated external feelings.The treatment should be regulating and harmonizing Ying-phase and Wei-phase,so that the internal and external functions of Wei-phase are normal to reduce the number of repeated attacks or external effects in children.Invigorating spleen and benefiting Qi,nourishing Yin and generating the body fluid methods can adjust the Yin-Yang bias of the Zang-Fu organs,and the harmonious state between Yin and Yang can enhance the immune function of children,forming the overall trea
