摘要
目的 分析某口腔专科医院出院患者31天内非计划再入院的原因及影响因素,为降低非计划再入院率提供理论依据。方法 对某三级甲等口腔医院2015年1月1日-2019年12月31日五年内19 682例出院患者住院病案首页资料进行描述性分析,将出院患者分为研究组(有31天内非计划再入院组)和对照组(无31天内非计划再入院组)进行对比研究,分析出院患者31天内非计划再入院的原因及影响因素。结果 2015年1月1日-2019年12月31日该医院出院患者19 682例,其中31天内非计划再入院患者88例,占出院患者的0.45%。出院患者31天内非计划再入院前三位原因分别是手术后伤口感染、手术后出血以及间隙感染。单因素回归分析结果表明,年龄、住院天数、手术级别,以及是否手术等因素具有统计学意义。其中住院天数、是否手术是出院患者31天内非计划再入院的独立影响因素。结论 非计划再入院是医疗质量的体现,该院31天内非计划再入院原因主要是手术后相关并发症,其中住院天数以及是否手术影响31天内非计划再入院率。
Objectives The causes and influencing factors of unplanned readmission of discharged patients within 31 days in a specialized hospital were analyzed to provide theoretical basis for reducing the unplanned readmission rate. Methods A descriptive analysis was made on the front page of the medical records of 19682 patients discharged from a stomatological hospital within five years from January 1st, 2015 to December 31st, 2019.Patients were divided into study group(unplanned readmission group within 31 days) and control group(no unplanned readmission group within 31 days) for comparative study. To analyze the causes and influencing factors of unplanned readmission within 31 days of discharge. Results A total of 19,682 patients were discharged from our hospital from January 1st, 2015 to December 31st, 2019, among which 88 patients were readmitted unplanned within 31 days, accounting for 0.45% of the discharged patients. The top three reasons for unplanned readmission within 31 days of discharge were postoperative wound infection, postoperative bleeding, and interspace infection.Univariate regression analysis showed that age, length of stay, level of surgery, and whether surgery was performed were statistically significant. The length of hospital stay and operation were independent influencing factors of unplanned readmission within 31 days. Conclusions Unplanned readmission was a reflection of medical quality. The main reasons for unplanned readmission within 31 days in this hospital were related complications after surgery,including length of stay and whether surgery affected the rate of unplanned readmission within 31 days.
