摘要
目的 分析医院工作人员报告医疗安全不良事件的影响因素、报告方式及其激励措施,提高不良事件报告率。方法 2021年3月15日-2021年4月14日,整群抽取某三级甲等医院的医师、护士、药师、技师、行政后勤管理人员等1030名研究对象,采用自填式电子问卷调查对不良事件报告进行现状调查,问卷内容包括基本信息、报告意愿、对不良事件的认知水平、软件的便捷性、激励措施等。采用SPSS25.0进行数据分析,经二元逻辑回归先后进行单因素分析和多因素分析,并对报告方式和鼓励报告的措施进行频数分析。结果 实际发放电子问卷1030份,回收有效问卷930份,有效问卷率90.3%。对不良事件报告的范围和时限的认知水平、现行报告系统的使用感是报告意愿的独立影响因素;45.7%的调查对象倾向信息系统(PC端)的报告方式;50.2%的调查对象选择医疗质量积分奖励的激励措施。结论 医院工作人员对医疗安全不良事件的报告范围和时限的认识有限,现行系统使用感不佳影响其报告的意愿。建议从认知宣教培训,升级信息系统和奖励质量安全积分等措施入手,提高不良事件报告率。
Objectives This study aims to analyze the influencing factors, reporting approaches and incentive measures of healthcare personnel reporting the medical safety adverse events, in order to enhance the reporting rate of adverse event. Methods From March 15, 2021 to April 14, 2021, 1030 subjeets including doctors, nurese,pharmacists, inspection and imaging technicians, administratire and logistics managers from a class Ⅲ class A hospital were selected in a cluster. Self-administrated electronic survey was used to investigate the adverse events in a tertiary hospital. The survey content included basic information, reporting willingness, cognitive level towards adverse events, software conveniency, incentive measures and so on. SPSS25.0 was used for data analysis. Binary logistic regression was performed for single factor analysis and multi factor analysis. Frequency analysis was used for investigating the reporting approaches and incentive measures. Results 1030 electronic questionnaires were handed out and 390 valid questionnaires were collected. Questionnaire validity rate was 90.3%. The independent influencing factors of reporting willingness were the cognitive level towards the scope and time limit of reporting adverse events and the sense of use towards the reporting system currently in use. 45.7% of the respondents preferred to use the reporting approach of information system(PC terminal). 50.2% of the respondents chose the incentive measure of medical quality reward points. Conclusions Healthcare personnel have a limited understanding of the reporting scope and time limit of medical safety adverse events. The low sense of use of current reporting system affected people’s reporting willingness. This study suggests that the reporting rate of adverse events can be enhanced by cognitive training of propaganda and education, system upgrade and quality safety reward points.
