摘要
目的探讨低剂量CT扫描联合肿瘤标志物检测对早期肺癌的筛查价值。方法选取我院影像科查体发现的60例肺结节作为观察对象,经穿刺、手术病理结果证实作为诊断依据,将肺癌32例作为观察组,良性结节28例为对照组。对比两组患者的CT征象及血清肿瘤标志物的水平,分析CT征象及血清肿瘤标志物诊断肺癌的价值,通过ROC曲线分析CT征象及血清肿瘤标志物联合检测对肺癌的诊断价值。结果两组患者在年龄、性别无差异,在病灶直径、边缘、胸膜凹陷征、空泡征、空气支气管征等CT征象差异有统计学意义(P<0.05)。观察组CEA、NSE、SCCA明显高于对照组(P<0.05)。CT诊断肺癌的准确率70.00%,灵敏度75.00%,特异度64.29%。肿瘤标志物诊断肺癌的准确率66.67%,灵敏度62.50%,特异度71.43%。CT联合肿瘤标志物检测诊断肺癌的准确率81.67%,灵敏度81.25%,特异度82.14%。CT肿瘤标志物联合检测诊断肺癌价值大于CT、肿瘤标志物检测单独诊断肺癌的价值,ROC曲线下面积分别为:CT 0.742(95%CI:0.609~0.874)、肿瘤标志物0.606(95%CI:0.930~0.980)、联合检测0.823(95%CI:0.708~0.939)。结论低剂量CT扫描联合肿瘤标志物检测对早期肺癌的筛查具有较大价值,值得推广。
Objective To investigate the screening price of low-dose CT scan combined with tumor markersin the detection for lung cancer.Methods 60 cases of lung nodules that were found in the imaging department of our hospital were selected as the research object,and the results confirmed by puncture and surgery were the basis for diagnosis,and 32 cases of lung cancer were assigned to observation group and 28 cases of benign nodules were enrolled in the control group.The levels of CT signs and serum tumor markers in the two groups were compared,the value of CT signs and serum tumor markers in diagnosing lung cancer was analyzed,and the diagnostic value of CT signs and serum tumor markers combined with CT signs and serum tumor markers was analyzed by ROC curve.Results There were no differences in age and sex between the two groups,and there were statistically significant differences in CT signs such as lesion diameter,margin,pleural depression,vacuole sign,and air bronchial sign(P<0.05).TheCEA,NSE,and SCCA in observation group were significantly higher than those in the control group(P<0.05).The correct rate of CT diagnosis of lung cancer was 70.00%,sensitivity was 75.00%and specificity was 64.29%.The correct rate of diagnosis of lung cancer by tumor markers was 66.67%,sensitivity was 62.50%and specificity was 71.43%.The correct rate of CT combined tumor marker detection and diagnosis of lung cancer was 81.67%,sensitivity was 81.25%and specificity was 82.14%.The value of CT tumor markers combined detection to diagnose lung cancer was greater than that of CT and tumor marker detection alone,and the area under the ROC curve was CT 0.742(95%CI:0.609~0.874),and that by the tumor marker was 0.606(95%CI:0.930~0.980),and the joint detection was 0.823(95CI 0.708~0.939).Conclusion Low-dose CT scan combined with tumor marker detection has great value for early lung cancer screening,which is worth promoting.
