摘要
目的了解苏州地区非结核分枝杆菌(NTM)病的病原学特征。方法回顾性选取就诊于苏州市第五人民医院2014年1月至2019年1月期间确诊为NTM病患者,收集年龄、性别等基本信息,同时对留痰、肺泡灌洗液、脑脊液、尿沉渣、胸腹水等NTM阳性样本进行全基因测序,建立基因进化树分析菌株间同源性,对100%同源样本进行全基因组共线分析。结果苏州地区共发现NTM病例67例,病例由2014年的3例增加到2018年的22例,以胞内分枝杆菌为主(38株,56.7%)。基因进化树分析发现堪萨斯分枝杆菌各菌株间同源性偏高。同一患者的菌株基因共线性范围较高。结论苏州地区NTM病发现例数逐年增多,致病菌种以胞内分枝杆菌为主。今后需密切关注堪萨斯分枝杆菌在人际间传播的可能性。
Objective To understand the etiological characteristics of nontuberculous mycobacteria(NTM)infection in Suzhou.Methods Patients with NTM disease diagnosed in Suzhou Fifth People's Hospital from January 2014 to January 2019 were retrospectively enrolled.Clinical information about age,gender was collected,and whole gene sequencing was used to analyze NTM positive samples from sputum,alveolar lavage fluid,cerebrospinal fluid,urine sediment,chest and ascites.Then,the gene evolutionary tree was established to analyze the homology between strains,and the whole genome collinear analysis was carried out on 100%homologous samples.Results There were 67 patients with NTM disease in Suzhou.The number of cases increased from 3 in 2014 to 22 in 2018,and the majority was Mycobacterium intracellulare infection(38 strains,56.7%).The gene evolution tree revealed high homology among the strains of Mycobacterium Kansasii,and the gene collinearity range of strains from the same patient was high.Conclusions The number of patients with NTM disease in Suzhou is increasing year by year and the main pathogenic bacteria is Mycobacterium intracellulare.Close attention should be paid to the possibility of human-to-human transmission of Mycobacterium Kansasii.
