支原体肺炎合并肥胖患儿血清炎症因子及疾病严重程度观察 被引量:1

Observation of serum inflammatory factors and disease severity in obese children with mycoplasma pneumonia

摘要 目的观察支原体肺炎合并肥胖患儿血清炎症因子及疾病严重程度。方法本研究为病例对照研究。采用非随机抽样方法,选择2018年1月至2020年12月马鞍山十七冶医院收治的100例支原体肺炎合并肥胖患儿作为观察组,另于同期选择50例无肥胖症的支原体肺炎患儿作为对照组。检测所有患儿血清炎症因子[肿瘤坏死因子α(TNF-α)、白细胞介素2(IL-2)、IL-6、IL-8]水平及核转录因子κB(NF-κB)、NF-κB抑制蛋白(IκB)、Toll样受体4(TLR4)的mRNA相对表达量。比较观察组和对照组、轻症支原体肺炎和重症支原体肺炎患儿血清TNF-α、IL-2、IL-6、IL-8水平及NF-κB、IκB、TLR4 mRNA相对表达量;以Pearson相关系数检验炎症因子水平与NF-κB、IκB、TLR4 mRNA相对表达量的相关性。结果与对照组相比,观察组TNF-α、IL-6、IL-8水平较高,IL-2水平较低,NF-κB mRNA、TLR4 mRNA相对表达量较高,IκB mRNA相对表达量较低(P值均<0.05)。与轻症支原体肺炎患儿相比,重症支原体肺炎患儿TNF-α、IL-6、IL-8水平较高,IL-2水平较低,NF-κB mRNA、TLR4 mRNA相对表达量较高,IκB mRNA相对表达量较低(P值均<0.05)。经相关性分析,TNF-α、IL-6、IL-8水平与NF-κB mRNA、TLR4 mRNA相对表达量呈正相关,与IκB mRNA相对表达量呈负相关;IL-2水平与NF-κB mRNA、TLR4 mRNA相对表达量呈负相关,与IκB mRNA相对表达量呈正相关(P值均<0.05)。结论支原体肺炎患儿合并肥胖症易导致NF-κB、TLR4 mRNA相对表达量及TNF-α、IL-6、IL-8等炎症因子水平升高,引发肺部炎症反应。此外,随着疾病严重程度的加重,炎症反应也逐渐加重。 Objective To observe the serum inflammatory factors and disease severity of obese children with mycoplasma pneumonia.Methods This was a case-control study.Non-random sampling was used.A total of 100 children with mycoplasma pneumonia complicated with obesity who were admitted to Maanshan Shiqiye Hospital from January 2018 to December 2020 were included in the observation group as the research subjects,and 50 children with mycoplasma pneumonia without obesity during the same period were selected as the control group.Serum inflammatory factors[TNF-α,interleukin-2(IL-2),IL-6,and IL-8]expression levels in all children were detected.The mRNA relative expression levels of nuclear transcription factor-κB(NF-κB),NF-κB inhibitory protein(IκB),and Toll-like receptor 4(TLR4)were detected.The levels of TNF-α,IL-2,IL-6,IL-8,and the mRNA relative expression levels of NF-κB,IκB,and TLR4 were compared between the observation group and the control group,and between mild and severe mycoplasma pneumonia.The correlation between the level of inflammatory factors and the mRNA relative expression levels of NF-κB,IκB,and TLR4 was tested by Pearson correlation coefficient.Results Compared with the control group,the levels of TNF-α,IL-6,and IL-8 were higher,the level of IL-2 was lower,the mRNA relative expression levels of NF-κB and TLR4 were higher,and the mRNA relative expression level of IκB was lower in the observation group(all P<0.05).Compared with mild mycoplasma pneumonia,the levels of TNF-α,IL-6,and IL-8 were higher,the level of IL-2 was lower,the mRNA relative expression levels of NF-κB and TLR4 were higher,and the mRNA relative expression level of IκB was lower in severe mycoplasma pneumonia(all P<0.05).Correlation analysis showed that TNF-α,IL-6,and IL-8 levels were positively correlated with the mRNA relative expression levels of NF-κB and TLR4,and negatively correlated with the relative expression level of IκB mRNA(all P<0.05).IL-2 level was negatively correlated with the mRNA relative expression levels of
