摘要
目的分析高频超声联合尿酸(UA)、C反应蛋白(CRP)、环氧化酶-2(COX-2)检测在痛风性关节炎(GA)诊断中的应用价值。方法收集2019年3月—2021年4月本院收治的127例GA作为GA组,另选取本院同期体检健康者109例作为健康组及因关节肿痛就诊的非GA患者115例作为对照组。比较3组血清UA、CRP及COX-2水平,观察GA组与对照组高频超声特征,绘制受试者工作特征(ROC)曲线分析高频超声联合UA、CRP、COX-2检测对GA的诊断价值。结果GA组UA、COX-2、CRP水平明显高于对照组与健康组(P<0.05)。GA组痛风石、双边征、暴雪征、肌腱周围强回声阳性占比明显高于对照组(P<0.01)。ROC曲线分析显示,高频超声联合UA、CRP、COX-2检测诊断GA的敏感度和特异度分别为0.966、0.837,曲线下面积为0.849(95%CI为0.763,0.914),明显高于高频超声、UA、CRP、COX-2单独检测。结论高频超声联合UA、CRP、COX-2检测可提高GA诊断敏感度、特异度,可为临床制定早期干预措施提供重要参考信息。
Objective To analyze the application value of high frequency ultrasound(HFU)combined with uric acid(UA),Creactive protein(CRP)and cyclooxygenase-2(COX-2)in diagnosis of patients with gouty arthritis(GA).Methods A total of 127 patients with GA admitted between March 2019 and April 2021 were collected as GA group,and 109 healthy people who had undergone physical examinations at the same period were selected as healthy group,and 115 nonGA patients who had visited doctors for joint swelling and pain were as control group.Serum levels of UA,CRP and COX-2 were compared,and characteristics of HFU were observed in GA and control groups.Receiver operating characteristic(ROC)curve was drawn to analyze the value of HFU combined with UA,CRP and COX-2 in diagnosis of GA.Results Serum levels of UA,COX-2 and CRP in GA group were significantly higher than those in control and healthy groups(P<0.05).The positive rates of gouty tophus,double contour sign,Blizzard sign and peritendon hyperecho in GA group were significantly higher than those in control group(P<0.01).ROC curve analysis showed that the sensitivity and specificity of HFU combined with UA,CRP and COX-2 in diagnosis of GA were 0.966 and 0.837 respectively,and the area under the ROC curve was 0.849(95%CI:0.763,0.914),which was significantly higher than those by HFU,UA,CRP and COX-2 detection alone.Conclusion HFU combined with UA,CRP and COX-2 may improve the sensitivity and specificity in diagnosis of GA,and it may provide important reference information for early intervention in clinical practice.
