摘要
目的探讨护理不良事件中第二受害者护士的心理体验,为构建针对性支持体系提供参考,从而促进护士和患者安全。方法采用质性研究中的现象学研究方法,于2019年2月-12月选取北京市某三甲医院15名经历过不良事件的护士,通过半结构深入访谈收集资料,采用Colaizzi 7步分析法进行资料分析。结果提炼出4个主题:(1)情绪体验。包括担忧、愧疚与自责、焦虑;(2)认知体验。包括怀疑自身能力、从事件中成长;(3)应对行为体验。包括以问题为中心的应对和以情绪为中心的应对;(4)支持需求。包括切实可行的预防措施和建立非责备性文化。结论护士第二受害者身心负担较为严重。护理管理者应关注第二受害者的体验,了解其支持需求,建立第二受害者相关支持系统,给予指导和干预,以改善护士第二受害者的身心健康状况,保障护士和患者安全。
Objective To explore the psychological experience of the second victim nurse in nursing adverse events,and provide reference for the constr uction of targeted support system,so as to promote the safety of nurses and patients.Methods The phenomenological research method of qualitative research Was used to recruit 15 nurses who had experienced adverse events in a tertiary hospital in Beiing from February to December 2019.Data were collected through semi-structured in-depth interviews and analyzed by Colaizzi 7-step analysis method.Results The research had extracted four themes:(1)emotion experience.lncluding worry,guilt and self-blame,anxiety;(2)cognition experience.In-cluding doubting self-ability,and growth from events;(3)coping behavior experience.Including problem-centered coping and emotion:centered coping;(4)support needs.Including practical preventive measures and the establishment of a non-blame culture.Conclusion The second victim nurse had a serious physical and mental burden.Nursing managers should pay attention to the experience of the second victim,understand their support needs,establish the relevant support system for the second victim,give guidance and intervention,so as to improve the physical and mental health of the nurses of the second victim,and ensure the safety of nurses and patients.
