摘要
目的分析连续性护理干预在系统性红斑狼疮合并糖尿病患者中的应用效果及对心理状态及并发症的影响。方法选取2019年1月—2021年1月福建医科大学附属第一医院收治的80例系统性红斑狼疮合并糖尿病患者为研究对象,依据不同护理干预方法将患者分为对比组(常规护理)、研究组(连续性护理),每组40例。观察评估两组血糖指标、心理状态、并发症发生率及生活质量。结果干预后,研究组患者空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)水平及焦虑自评量表(SAS)和抑郁自评量表(SDS)评分较对比组更低,差异有统计学意义(P<0.05)。研究组感染、血管炎、低血糖及消化道损伤等并发症总发生率10.00%较对比组30.00%更低,差异有统计学意义(P<0.05)。干预后,研究组生活质量综合评定问卷(GQOL-74)4个维度评分均较对比组更高,差异有统计学意义(P<0.05)。结论对系统性红斑狼疮合并糖尿病患者实施连续性护理干预,可更好地降低血糖水平、改善患者心理状态,且对降低并发症发生率、提高生活质量具有积极影响。
Objective To analyze the application effect of continuous nursing intervention in patients with systemic lupus erythematosus complicated with diabetes mellitus and its influence on psychological state and complications.Methods 80 patients with systemic lupus erythematosus complicated with diabetes treated in the First Affiliated Hospital of Fujian Medical University from January 2019 to January 2021 were selected as the research objects.According to different nursing intervention methods,the patients were divided into control group(routine nursing)and study group(continuous nursing),with 40 cases in each group.The blood glucose index,psychological status,complication rate and quality of life of the two groups were observed and evaluated.Results After the intervention,the levels of fasting blood glucose(FPG),2 h postprandial blood glucose(2 hPG),glycosylated hemoglobin(HbA1c),Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)in the study group were lower than those in the control group,and the difference was statistically significant(P<0.05).The total incidence of complications such as infection,vasculitis,hypoglycemia and gastrointestinal injury in the study group was 10.00%,which was lower than 30.00%in the control group,and the difference was statistically significant(P<0.05).After the intervention,the scores of Comprehensive Assessment Questionnaire for Quality of Life(GQOL-74)in the study group were higher than those in the control group,and the difference was statistically significant(P<0.05).Conclusion Implementing continuous nursing intervention for patients with systemic lupus erythematosus complicated with diabetes can better reduce blood glucose level and improve the psychological state of patients.And it has a positive impact on reducing the incidence of complications and improving the quality of life.
