摘要
目的探讨TyG及TyG-BMI与2型糖尿病血糖管理情况的相关性。方法选择2019年1月—2021年12月在国药东风花果医院住院的222例2型糖尿病患者,按照糖化血红蛋白目标值(7%)分为血糖良好组(84例)和血糖欠佳组(138例)。收集两组患者临床资料及空腹血糖、血生化、血脂等相关检验指标,按公式计算TyG及TyG-BMI指数,使用相关分析及Logistic回归分析上述指标与糖化血红蛋白的关系。结果两组糖尿病并发症、合并症(脑梗塞、高血压)、SBP、尿酸、TG、TC、LDL-C、FPG、TyG以及TyG-BMI对比,差异有统计学意义(P<0.05)。2型糖尿病患者TyG、TyG-BMI与HbA1c呈正相关(r=0.298、0.159,P<0.05)。糖尿病患者TG、TC、LDL-C、FPG、尿酸、胱抑素C与TyG具有相关性(P<0.001),糖尿病患者体质量、BMI、TG、HDL-C、FPG、胱抑素C与TyG-BMI具有相关性(P<0.05)。Logistic回归分析显示TyG及TyG-BMI升高是血糖管理不良的独立危险因素(P<0.05);ROC曲线分析提示在预测血糖管理情况上,TyG、TyG-BMI都有较好的性能(P<0.05),其中TyG表现更为突出。结论TyG及TyG-BMI指数可作为2型糖尿病患者血糖管理的观察指标。
Objective To investigate the correlation of TyG and TyG-BMI with blood glucose management in type 2diabetes.Methods A total of 222 patients with type 2 diabetes who were hospitalized in Sinopharm Dongfeng Huaguo Hospital from January 2019 to December 2021 were selected and divided into a good blood sugar group(84 cases)and a poor blood sugar group(138 cases)according to the target glycated hemoglobin value(7%).The clinical data,fasting blood glucose,blood biochemistry,blood lipids and other related test indexes of the two groups of patients were collected,and the TyG and TyG-BMI indexes were calculated according to the formula.The correlation analysis and logistic regression were used to analyze the relationship between the above indexes and glycosylated hemoglobin.Results There were statistically significant differences in diabetes complications,complications(cerebral infarction,hypertension),SBP,uric acid,TG,TC,LDL-C,FPG,TyG and TyG-BMI between the two groups(P<0.05).TyG,TyGBMI and HbA1c were positively correlated in patients with type 2 diabetes(r=0.298,0.159,P<0.05).There was a correlation between TG,TC,LDL-C,FPG,uric acid,cystatin C and TyG in diabetic patients(P<0.001).The body mass,BMI,TG,HDL-C,FPG,cystatin C and TyG-BMI were correlated in diabetes patients(P<0.05).Logistic regression analysis showed that the increase of TyG and TyG-BMI were independent risk factors for poor blood glucose management(P<0.05);ROC curve analysis showed that TyG and TyG-BMI had better performance in predicting blood glucose management(P<0.05),and TyG was more prominent.Conclusion TyG and TyG-BMI index can be used as observation indicators of blood glucose management in patients with type 2 diabetes.
