南京市2019年哨点医院腹泻重点病原菌药敏监测结果分析 被引量:2

Analysis on drug susceptibility monitoring results of key pathogens for diarrhea from sentinel hospitals of Nanjing in 2019

摘要 目的 了解南京市2019年腹泻重点病原菌药敏监测结果及耐药特征。方法 收集南京市2019年哨点医院腹泻门诊患者粪便标本,从中分离得到沙门菌和致泻性大肠埃希菌菌株,使用药敏分析系统对菌株药物敏感性进行检测,并进一步分析其耐药特征。结果 南京地区2019年沙门菌普遍对亚胺硫霉素和阿奇霉素敏感,耐药率较高的抗生素依次为氨苄西林(60.34%)、头孢唑啉(56.90%)、氨苄西林/舒巴坦(53.45%)、四环素(50.00%),同时发现多株多重耐药菌株。致泻性大肠埃希菌对氨苄西林、四环素、庆大霉素、头孢噻肟、头孢唑啉和萘啶酸耐药,耐药率分别为61.11%、44.44%、33.33%、33.33%、33.33%、33.33%,对亚胺硫霉素和头孢他啶均敏感不耐药。结论 南京地区2019年沙门菌和致泻性大肠埃希菌存在较为严重的抗生素耐药。需加强对腹泻致病菌的药敏监测,结合致病菌的耐药性进行针对性用药。 Objective To understand the drug susceptibility monitoring results and antibiotic resistance characteristics of key pathogens for diarrhea from sentinel hospitals of Nanjing in 2019.Methods The fecal specimens of patients from diarrhea outpatient departments of sentinel hospitals of Nanjing in 2019 were collected, Salmonella and diarrheogenic Escherichia coli were isolated from the patients, the drug susceptibility analysis system was applied to detect the drug susceptibility of bacterial strains, and the characteristics of their antibiotic resistance were further analyzed.Results Salmonella species in Nanjing region in 2019 were generally sensitive to imipenem and azithromycin, and the antibiotics with high drug resistance were ampicillin(60.34%),cefazolin(56.90%),ampicillin/sulbactam(53.45%) and tetracycline(50.00%) successively, and several multi-drug resistant strains were also found.The drug resistance rate of diarrheogenic Escherichia coli to ampicillin, tetracycline, gentamicin, cefotaxime, cefazoline and naphthalenic acid was 61.11%,44.44%,33.33%,33.33%,33.33% and 33.33%,respectively.The diarrheogenic Escherichia coli were sensitive but not drug-resistant to imipenem and ceftazidime.Conclusion Salmonella and diarrheagenic Escherichia coli in Nanjing region are found to have severe antibiotic resistance in 2019.It is required to strengthen the monitoring on the drug susceptibility of pathogenic bacteria for diarrhea and take targeted medication by combining the drug resistance characteristics of pathogenic bacteria.
