摘要
目的:探究阴道超声在早期宫外孕诊断中的效果。方法:选取东阿县人民医院2021年1月—2022年1月收治的疑似早期宫外孕产妇47例,均进行阴道超声诊断、腹部超声诊断,以病理诊断结果作为金标准,比较两种诊断方式的检出结果、诊断效率及具体鉴别情况。结果:47例产妇中,经病理诊断确诊为早期宫外孕共计46例;阴道超声诊断确诊43例(91.48%),漏诊3例(6.38%);腹部超声诊断确诊38例(80.85%),漏诊8例(17.02%)。诊断效能方面,阴道超声和腹部超声诊断的特异度均为100.00%,但是前者的准确度(93.61%)和敏感度(93.47%)均较后者(82.97%、82.60%)更高。阴道超声方式诊断的附件包块、假孕囊、盆腔积液的检出率均显著高于腹部超声检查(P<0.05)。在包块内部检查中,阴道超声对妊娠囊、卵黄囊、胚芽、胎心搏动的检出率均显著高于腹部超声(P<0.05)。结论:阴道超声在早期宫外孕的诊断中具有较为理想的诊断价值,相较腹部超声诊断,检出率更高,还可较好地鉴别宫外孕类型,值得推广运用。
Objective To explore the effect of transvaginal ultrasound in the diagnostic of early ectopic pregnancy.Methods Forty-seven cases of pregnant women with suspected early ectopic pregnancy admitted to Dong'e County People's Hospital from January 2021 to January 2022 were selected.All of them were diagnosed by transvaginal ultrasound and abdominal ultrasound,and pathological diagnosis results were used as the gold standard to compare the detection results,diagnostic efficiency and specific discrimination of the two diagnostic methods.Results Among 47 parturients,46 cases were diagnosed as early ectopic pregnancy by pathological diagnosis.Transvaginal ultrasonography confirmed 43 cases(91.48%)and missed 3 cases(6.38%).Abdominal ultrasonography confirmed 38 cases(80.85%)and missed 8 cases(17.02%).In terms of diagnostic efficiency,the specificity of vaginal ultrasound and abdominal ultrasound were both 100.00%,but the accuracy(93.61%)and sensitivity(93.47%)of the former were higher than that of the latter(82.97%,82.60%).The detection rates of adnexal mass,pseudopregnancy sac and pelvic effusion diagnosed by transvaginal ultrasound were significantly higher than those diagnosed by abdominal ultrasound(P<0.05).In the internal examination of mass,the detection rate of pregnancy sac,yolk sac,embryo and fetal heart beat by transvaginal ultrasound was significant higher than that by abdominal ultrasound(P<0.05).Conclusion Transvaginal ultrasound is useful in the diagnosis of early ectopic pregnancy.It has ideal diagnostic value.Compared with abdominal ultrasonography,the detection rate is higher,and it can better identify the types of ectopic pregnancy.It is worthy of popularization and application.
