磁共振成像检测颞叶癫痫患者不同脑区脑白质纤维束FA、MD值及分析皮层厚度与认知功能的相关性 被引量:1

Association of FA,MD values of white matter fiber bundles in different brain regions detected by MRI and cortical thickness with cognitive function of patients with epilepsy

摘要 目的 探讨磁共振成像检测颞叶癫痫患者不同脑区脑白质纤维束FA及MD值、分析皮层厚度与认知功能的相关性。方法 32例左侧颞叶癫痫患者为癫痫组,32例同期健康体检者为对照组,采用简易智力状态检查量表(MMSE)进行认知功能评估、弥散张量成像(DTI)检查及测定皮层厚度,比较两组被检者认知功能、影像学检查感兴趣区各向异性分数(FA)值、平均弥散率(MD)值及皮层厚度,分析认知功能与皮层厚度之间的关系。结果 癫痫组的MMSE各项评分均低于对照组(P<0.05);癫痫组左侧扣带回FA值较对照组低,差异具有统计学意义(P<0.05);癫痫组双侧海马、双侧扣带回、穹隆、左侧钩回、双侧外囊MD值均高于对照组,差异具有统计学意义(P<0.05);癫痫组左侧颞下回、左侧楔前叶、左侧扣带回皮层变薄,差异有统计学意义(P<0.05);左侧楔前叶和定向力(r=3.954)、左侧楔前叶和视空间(r=3.714)呈正相关关系,左侧颞下回和延迟回忆呈正相关关系(r=2.965,P<0.05)。结论 颞叶癫痫患者存在认知功能障碍,DTI和皮层厚度测定可有效评估癫痫患者神经脑组织受损情况。 Objective To study the association of fractional anisotropy( FA), mean diffusivity(MD) and cortical thickness with cognitive function of patients with epilepsy.Methods Thirty-two patients with left temporal lobe epilepsy(LTLE) were selected as epilepsy group, and 32 healthy cases were selected as control group.Mini-Mental State Examination( MMSE) was applied to assess cognitive function.Two groups were given with diffusion tensor imaging(DTI).Cortical thickness was determined.Cognitive function, FA value of the area of interest in imaging examination, MD value and cortical thickness were compared between two groups.The correlation between cognitive function and cortical thickness was analyzed.Results MMSE scores of the epilepsy group were lower than those of control group(P< 0.05).FA value in the left cingulate gyrus of epilepsy group was lower than that of control group(P< 0.05).MD values in bilateral hippocampus, bilateral cingulate gyrus, fornix, left uncinate gyrus, and bilateral external capsule in epilepsy group were higher than those in control group(P< 0.05).Left inferior temporal gyrus, left precuneus, and left cingulate cortex in epilepsy group was thinner than those in control group(P< 0.05).Left precuneus was positively correlated with directional force(r=3.954) and optic space(r=3.714).Left inferior temporal gyrus was positively associated with delayed recall(r=2.965,P< 0.05).Conclusion Patients with LTLE have cognitive dysfunction.DTI and cortical thickness will be used to effectively assess damages of neural and brain tissues in patients with epilepsy.
