氧气驱动雾化吸入联合无创呼吸机治疗慢性阻塞性肺疾病合并呼吸衰竭患者的效果 被引量:3

摘要 目的:观察氧气驱动雾化吸入联合无创呼吸机治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的效果。方法:选取2018年4月至2020年4月该院收治的80例COPD合并呼吸衰竭患者进行前瞻性研究,按照随机数字表法分为对照组和观察组各40例。对照组给予无创呼吸机治疗,观察组在对照组基础上联合氧气驱动雾化吸入治疗,比较两组治疗前后血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、pH值]水平,肺功能指标[用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、FEV_(1)/FVC]水平,呼吸问题[圣乔治医院呼吸问题调查问卷(SGRQ)]评分及并发症发生率。结果:治疗后,观察组PaO_(2)水平、pH值均高于对照组,PaCO_(2)水平低于对照组,差异有统计学意义(P<0.05);观察组FVC、FEV_(1)及FEV_(1)/FVC水平均高于对照组,差异有统计学意义(P<0.05);观察组疾病影响、活动能力及呼吸症状等各项SGRQ评分均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率为5.00%,低于对照组的20.00%,差异有统计学意义(P<0.05)。结论:无创呼吸机联合氧气驱动雾化吸入治疗COPD合并呼吸衰竭患者,可改善血气指标水平,促进肺功能恢复,改善呼吸问题,且安全性高,其效果优于单纯无创呼吸机治疗。 Objective:To observe effects of oxygen drive atomization inhalation combined with noninvasive ventilator in the treatment of chronic obstructive pulmonary disease(CO_(2)PD) patients with respiratory failure.Methods:80 patients with CO_(2)PD and respiratory failure who were admitted to this hospital from April 2018 to April 2020 were selected for the prospective study,and were divided into control group and observation group according to the random number table method,40 cases in each group.The control group was given noninvasive ventilator therapy,while the observation group was given oxygen drive atomization inhalation therapy on the basis of that of the control group.The levels of blood gas analysis indexes [arterial partial pressure of oxygen(PaO),arterial carbon dioxide partial pressure(PaCO_(2)),pH value],the pulmonary function index levels [forced vital capacity(FVC),maximum expiratory volume in 1 second(FEV_(1)),the rate in 1 second(FEV_(1)/FVC)],the respiratory problems [St.George’s Hospital respiratory problem questionnaire(SGRQ)] score and the incidence of complications were compared between the two groups before and after the treatment.Results:After the treatment,the PaOand pH values of the observation group were higher than those of the control group;the level of PaCO_(2)was lower than that of the control group;and the differences were statistically significant(P<0.05).The levels of FVC,FEV_(1)and FEV_(1)/FVC in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The SGRQ scores of disease impact,activity ability and respiratory symptoms in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Further,the incidence of complications in the observation group was 5.00%,which was lower than 20.00% in the control group,and the difference was statistically significant(P<0.05).Conclusions:Noninvasive ventilator combined with oxygen drive atomization inhalation in th
