摘要
目的探讨手部遇水膨胀剂高压注射伤的显微外科治疗效果。方法回顾性分析2017年8月-2021年5月治疗的手部遇水膨胀剂手部高压注射伤患者5例,入院后通过急诊清创结合显微镜下彻底清除遇水膨胀剂细微颗粒和其他异物,术后置引流管引流,配合积极康复训练。结果本组5例患者,术后通过门诊及微信随访6~26个月,平均11个月,手部活动功能良好无指体坏死及皮肤坏死。末次随访根据TAM(手指主动活动度)评定法评价患手功能:优3例,良1例,可1例。结论手部遇水膨胀剂高压注射伤条件允许时应急诊彻底清创,并在手术显微镜下彻底清除遇水膨胀剂颗粒和其他异物,清创完毕后置引流管引流,一周后观察伤口周围清洁无明显感染征象时拔除引流管,结合早期积极训练,可获得良好的疗效。
Objective To discuss the therapentic effect of microsurgical treatment of hand pressure injection injury caused by water expansion agent.Methods Five cases of hand surgery in our hospital were retrospectively analyzed from August 2017 to May 2021 to treat hand high pressure injection injury cansed by water expansion agent and the atment of hand pressure injection injury.After admission,the fine particles and other foreign bodies of the water expansion agent were thoroughly removed under the emergency cleaning and opening microscope.After operation drainage tube drainage,cooperated with active rehabilitation exercise.Results In the group,5 patients were followed up in outpatient and weixin follow-up for 6-26 months after surgery,with an avaverage of 11 months.The hand activity function was good without finger necrosis or skin necrosis,the last follow-up was evaluated by the evaluation method of TAM(finger active activity):3 cases were excellent,1 was good,1 was fair.Conclusion The condition of high pressure injection injury caused by water expansion agent is allowed to complete the emergency diagnosis and thoroughly remove the injected granule and other foreign bodies under the surgical microscope,after the cleaning up,the drainage pipe is indwelled,after a week,the drainage pipe is removed when cleaning around the wound is observed without obvious infection signs,combined with early active exercise injury ability,good efficacy can be obtained.
