摘要
目的观察阳经腧穴联合神经生理疗法对急性脑梗死(ACI)恢复期患者吞咽功能、自理能力的影响。方法选取2017年10月—2020年10月在四川大学华西医院就诊的ACI恢复期患者152例,按照随机数字表法分为对照组和联合组,每组各76例。对照组采用针灸阳经腧穴进行治疗,联合组在对照组的基础上行神经生理疗法,两组患者共治疗2个月。于治疗前、治疗1个月后、治疗2个月后检测神经因子相关指标[神经元特异性烯醇化酶(NSE)、中枢神经特异蛋白(S100β)、神经生长因子(NGF)]及氧化应激指标[丙二醛(MDA)、谷胱甘肽过氧化物酶(GSHPx)、超氧化物歧化酶(SOD)],治疗2个月后评定患者吞咽功能(Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级及Ⅴ级)、自理能力(无需依赖、轻度依赖、中度依赖及重度依赖),统计治疗效果。结果与对照组相比,联合组治疗2个月后Ⅰ、Ⅱ级率,无需依赖率及治疗总有效率均升高,Ⅲ、Ⅳ、Ⅴ级率,轻度、中度、重度依赖率均降低。与治疗前相比,两组患者治疗1个月后、2个月后NGF、GSH-Px、SOD水平均升高,NSE、S-100β、MDA水平均降低;治疗后与同时期对照组相比,联合组NGF、GSH-Px、SOD水平均升高,NSE、S-100β、MDA水平均降低,差异具有统计学意义(P<0.05)。结论阳经腧穴联合神经生理疗法可改善ACI恢复期患者吞咽功能、自理能力,提高患者治疗有效率。
Objective To study the effect of Yangjing acupoints combined with neurophysiological therapy on swallowing function and self-care ability of patients with acute cerebral infarction(ACI)in convalescence.Methods A total of 152 patients who visited west china hospital for ACI recovery from October 2017 to October 2020 were selected and divided into the control group and the combination group according to the random number table,76 cases in each group.The control group was treated with acupuncture Yangjing acupoints,The combined group performed neurophysiological therapy on the basis of the control group,The two groups were treated for 2 months.nerve factor-related indicators[neuron-specific enolase(NSE),central nervous specific protein(S100β),nerve growth factor(NGF)],oxidative stress indicators[malondialdehyde(MDA),glutathione peroxidase(GSH-Px),superoxide dismutase(SOD)].After 2 months of treatment,patients were assessed for swallowing function(Ⅰ,Ⅱ,Ⅲ,Ⅳ and Ⅴ),self-care(no dependence,mild dependence,moderate dependence and severe dependence),statistical treatment effect.Results Compared with the control group,after 2 months of treatment,the grade Ⅰ,Ⅱ rate,no dependence rate and total effective rate of the combined group were increased,while the grade Ⅲ,Ⅳ,Ⅴ rate,mild dependence,moderate dependence and severe dependence rate were decreased.Compared with before treatment,the levels of NGF,GSH-PX and SOD of the two groups were increased,and the levels of NSE,S-100β and MDA were decreased after 1 month and 2 months of treatment;compared with those of the control group after 1 month and 2 months of treatment.Compared with the control group,the levels of NGF,GSH-Px and SOD in the combined group increased after 1 month and 2 months of treatment,while the levels of NSE,S-100β and MDA decreased,with statistical difference(P<0.05).Conclusion Yangjing acupoints combined with neurophysiological therapy can improve the swallowing function,self-care ability and treatment efficiency of patients with ACI in r
