摘要
目的分析GDM患者外周血hs-CRP、SAA、RBP4水平与产后代谢综合征的关系。方法选取2018年1月至2020年1月中国人民解放军联勤保障部队第九二〇医院诊治的82例GDM患者(观察组)与53例健康孕妇(对照组)的临床资料进行回顾性分析,观察组根据产后1年内有无发生代谢综合征(MS)分为MS组(n=41)与非MS组(n=41),对比hs-CRP、SAA、RBP4水平,以及采用Logistic分析hs-CRP、SAA、RBP4表达量对产后代谢综合征的影响。结果观察组和MS组的hs-CRP、SAA、RBP4、BMI、FPG、SBP、TG表达量分别高于对照组与非MS组(P<0.05)。Pearson相关性分析显示,hsCRP与BMI、FPG、SBP、TG呈正相关(P<0.05);SAA与BMI、FPG、SBP、TG呈正相关(P<0.05);RBP4与BMI、FPG、SBP、TG呈正相关(P<0.05)。Logistic回归分析显示,hs-CRP、SAA、RBP4高表达为GDM患者发生MS的危险因素。ROC曲线分析显示,hs-CRP、SAA、RBP4预测MS发生的AUC分别为0.797、0.716、0.846(P<0.05),敏感度分别为51.20%、53.70%、70.70%;特异性分别为100.00%、100.00%、90.20%。结论外周血hs-CRP、SAA、RBP4在GDM患者中呈高表达,其水平的升高可能与产后MS的发生具有关联,可通过检测其含量为预测MS发生提供重要参考。
Objective To analyze the relationship between the levels of hs-CRP,SAA,RBP4 in peripheral blood and the postpartum metabolic syndrome in patients with gestational diabetes mellitus(GDM).Methods The clinical data of 82 GDM patients(observation group)and 53 healthy pregnant women(control group)who were diagnosed and treated from January 2018 to January 2020 were selected for retrospective analysis.The observation group was divided into MS group(n=41)and non-MS group(n=41)based on whether metabolic syndrome(MS)occurred within 1 year after delivery.The levels of hs-CRP,SAA and RBP4 were compared,and the logistic analysis was used to analyze the effects of expression of hs-CRP,SAA and RBP4 on the postpartum metabolic syndrome.Results The expression levels of hs-CRP,SAA,RBP4,BMI,FPG,SBP and TG in the observation group and MS group were higher than those in the control group and non-MS group(P<0.05).Pearson correlation analysis showed that hsCRP was positively correlated with BMI,FPG,SBP,and TG(P<0.05);SAA was positively correlated with BMI,FPG,SBP,and TG(P<0.05);RBP4 was positively correlated with BMI,FPG,SBP,and TG correlation(P<0.05).Logistic regression analysis showed that high expression of hs-CRP,SAA,and RBP4 were risk factors for MS in GDM patients.ROC curve analysis showed that hs-CRP,SAA,and RBP4 predicted the occurrence of MS with AUC(0.797,0.716,0.846,P<0.05),the sensitivity was 51.20%,53.70%,70.70%respectively,and the specificity was 100.00%,100.00%,90.20%respectively.Conclusions Peripheral blood hsCRP,SAA,and RBP4 are highly expressed in GDM patients.The increase in their levels may be related to the occurrence of postpartum MS.The detection of their levels can provide an important reference for predicting the occurrence of MS.
