摘要
目的通过观察急性缺血性卒中(AIS)患者的病灶多寡和中医证候要素的分布情况,探讨二者的相关性及其对临床辨证的意义。方法采用横断面研究方法,连续性收集157例AIS患者的一般资料、中医证候要素及影像学资料,根据梗死病灶的多寡分为单发病灶组(66例)和多发病灶组(91例)。通过描述性统计、单因素分析及Logistic回归分析研究AIS患者病灶多寡与中医证候要素的相关性。结果AIS中医证候要素分布依次为气虚(103例,65.61%)、痰湿(77例,49.04%)、内火(62例,39.49%)、内风(42例,26.75%)、血瘀(37例,23.57%)、阴虚(16例,10.19%)。Logistic回归分析显示,气虚与AIS患者多发病灶的发生呈正相关(P=0.009,OR=2.518,95%CI[1.260,5.029])。结论AIS患者的中医证候要素多为气虚,且气虚与多发病灶存在显著相关性,二者可能是通过改变血液流变学状态建立联系。
Objective To discuss the correlation and its significance to clinical syndrome differentiation between the number of lesions of acute ischemic stroke(AIS)and TCM syndrome factors by observing the number of lesions and the distribution of TCM syndrome elements in AIS patients.Methods The general data,TCM syndrome factors and imaging data of 157 AIS patients were collected continuously by cross-sectional study.According to the number of infarct lesions,the patients were divided into single lesion group(66 cases)and multiple lesion group(91 cases).Descriptive statistics,univariate analysis and Logistic regression analysis were used to study the correlation between the number of lesions in AIS patients and TCM syndrome factors.Results The distribution of TCM syndrome elements of AIS was qi deficiency(103 cases,65.61%),phlegm dampness(77 cases,49.04%),internal heat(62 cases,39.49%),internal wind(42 cases,26.75%),blood stasis(37 cases,23.57%),yin deficiency(16 cases,10.19%).Logistic regression analysis showed that qi deficiency syndrome was positively correlated with the occurrence of multiple lesions in AIS patients(P=0.009,OR=2.518,95%CI[1.260,5.029]).Conclusion The TCM syndrome elements of AIS patients are mostly qi deficiency,and there is a significant correlation between qi deficiency and multiple lesions,which may be established by changing the hemorheology state.
