摘要
目的:分析微创椎弓根螺钉内固定术治疗胸腰椎跳跃式骨折的临床效果。方法:选取2019年1月-2020年1月阳新县中医院收治的60例胸腰椎跳跃式骨折患者,随机分为两组,各30例。对照组行常规开放手术,观察组行微创椎弓根螺钉内固定术。比较两组患者治疗有效率、围术期指标情况(出血量、下床时间、住院时间)、并发症发生率、生活质量评分(情感功能、生理功能、躯体功能、社会功能)、病情改善情况[矢状面指数、伤椎前缘高度、Cobb角指标、日本骨科协会评估治疗分数(JOA)疼痛评分]。结果:观察组治疗总有效率高于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。观察组出血量少于对照组,下床时间及住院时间短于对照组,差异有统计学意义(P<0.05)。观察组情感功能、生理功能、躯体功能、社会功能评分均高于对照组,差异有统计学意义(P<0.05)。观察组矢状面指数、伤椎前缘高度、Cobb角指标、JOA疼痛评分均优于对照组,差异有统计学意义(P<0.05)。结论:微创椎弓根螺钉内固定术治疗胸腰椎跳跃式骨折的临床效果较好,能改善患者各手术指标,提高患者的生活质量,值得推广。
Objective:To analyze the clinical effect of minimally invasive pedicle screw internal fixation in the treatment of noncontiguous thoracolumbar fracture.Methods:A total of 60 patients with noncontiguous thoracolumbar fractures admitted to Yangxin County Hospital of Traditional Chinese Medicine from January 2019 to January 2020 were selected and randomly divided into two groups,with 30 cases in each group.The control group underwent conventional open surgery,while the observation group underwent minimally invasive pedicle screw internal fixation.The effective rate of treatment,perioperative indicators(blood loss,ambulation time,length of hospital stay),incidence of complications,quality of life scores(emotional function,physiological function,physical function,social function),and disease improvement[sagittal plane index,anterior edge height of injured vertebra,Cobb angle index,Japanese Orthopedic Association Assessment Score(JOA)pain score]were compared between the two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the incidence of complications was lower than that in the control group,and the difference was statistically significant(P<0.05).The amount of bleeding in the observation group was less than that in the control group,and the ambulation time and the length of hospital stay were shorter than those in the control group,and the differences were statistically significant(P<0.05).The scores of emotional function,physiological function,physical function and social function of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).The sagittal plane index,anterior edge height of injured vertebra,Cobb angle index and JOA pain score in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).Conclusion:The minimally invasive pedicle screw internal fixation in the treatment noncontiguous thoracolumbar fract
