摘要
目的探讨基于认知干预的早期持续被动锻炼结合间歇充气加压疗法在下肢开放性骨折术后患者中的应用效果。方法选取2019年8月至2021年8月我院80例下肢开放性骨折术后患者作为研究对象,以随机数字表法将其分为对照组和观察组,各40例。对照组实施常规护理,观察组在对照组基础上实施基于认知干预的早期持续被动锻炼结合间歇充气加压疗法。比较两组的干预效果。结果观察组的下肢深静脉血栓总发生率低于对照组(P<0.05)。干预后,观察组的股浅静脉、股总静脉、腘窝静脉血流速度高于对照组(P<0.05)。干预后,观察组的血栓调节蛋白(TM)、凝血酶-抗凝血酶复合物(TAT)及组织型纤溶酶原激活物-抑制剂复合物(t-PAIC)水平低于对照组(P<0.05)。结论对下肢开放性骨折术后患者实施基于认知干预的早期持续被动锻炼结合间歇充气加压疗法有助于提高下肢深静脉血栓的预防效果,促进静脉血流状态、凝血功能改善。
Objective To explore the application effect of early continuous passive exercise based on cognitive intervention combined with intermittent air pressure therapy in patients with open fracture of lower limb after surgery.Methods From August 2019 to August 2021,80 patients with open fracture of lower limb after surgery in our hospital were selected as the research objects,the patients were divided into control group and observation group by random number table method,with 40 cases in each group.The control group carried out routine nursing,while the observation group carried out early continuous passive exercise based on cognitive intervention combined with intermittent air pressure therapy on the basis of the control group.The intervention effects of the two groups were compared.Results The total incidence of lower extremity deep venous thrombosis in the observation group was lower than that in the control group(P<0.05).After intervention,the blood flow velocity of superficial femoral vein,common femoral vein and popliteal vein in the observation group were higher than those in the control group(P<0.05).After intervention,the levels of thrombomodulin(TM),thrombin-antithrombin complex(TAT)and tissue plasminogen activator inhibitor complex(t-PAIC)in the observation group were lower than those in the control group(P<0.05).Conclusion Early continuous passive exercise based on cognitive intervention combined with intermittent air pressure therapy for patients with open fracture of lower limb after surgery is helpful to improve the prevention effect of lower extremity deep venous thrombosis,promote the improvement of venous blood flow and coagulation function.
