巨细胞病毒感染对非酒精性脂肪性肝病血清miRNA-9和miRNA-122a水平及肝损伤的影响 被引量:4

Effects of cytomegalovirus infection on serum miRNA-9 and miRNA-122a levels and liver injury in patients with non-alcoholic fatty liver disease

摘要 目的探讨人巨细胞病毒(HCMV)感染对非酒精性脂肪性肝病(NAFLD)患者血清MicroRNA-9(miRNA-9)、MicroRNA-122a(miRNA-122a)水平和肝损伤的影响。方法选择2019年2月-2021年2月武汉理工大学医院、武汉市三医院120例NAFLD患者为NAFLD组,根据肝损伤程度分为轻度组47例、中度组40例、重度组33例。另选取同期在医院健康体检的50名健康者为健康对照组,均行肝功能、HCMV-DNA、血清特异性HCMV-免疫球蛋白(Ig)G、HCMV-IgM及miRNA-9、miRNA-122a相对表达量检测;分析NAFLD组不同肝损伤程度、不同HCMV感染情况患者血清miRNA-9、miRNA-122a表达水平差异,并进一步分析其相关性。结果NAFLD组天冬氨酸转移酶(AST)、丙氨酸转移酶(ALT)、直接胆红素(DBIL)、总胆红素(TBIL)、碱性磷酸酶(ALP)水平,HCMV-DNA、HCMV-IgG、HCMV-IgM阳性例数及血清miRNA-9、miRNA-122a相对表达量均高于健康对照组(P<0.05);轻度组、中度组、重度组HCMV-DNA阳性率、HCMV-IgM阳性率及血清miRNA-9、miRNA-122a相对表达量依次升高(P<0.05),三组HCMV-IgG阳性率比较,无统计学差异;Spearman相关性分析显示,NAFLD患者血清miRNA-9、miRNA-122a水平与肝损伤程度均呈正相关(P<0.05);NAFLD合并HCMV感染患者血清miRNA-9、miRNA-122a相对表达量均高于未感染者(P<0.05),NAFLD合并HCMV感染患者血清miRNA-9、miRNA-122a表达水平与HCMV-DNA含量及HCMV-IgM抗体滴度均呈正相关(P<0.05)。结论HCMV感染与NAFLD及肝损伤密切相关,可能通过影响miRNA-9、miRNA-122a的表达而参与疾病发展。 OBJECTIVE To investigate the effect of human cytomegalovirus(HCMV)infection on levels of serum MicroRNA-9(miRNA-9)and MicroRNA-122 a(miRNA-122 a)and liver injury in patients with non-alcoholic fatty liver disease(NAFLD).METHODS A total of 120 patients with NAFLD in Wuhan University of Technology Hospital and Wuhan Third Hospital from Feb.2019 to Feb.2021 were selected as the NAFLD group and divided into 47 cases of mild group,40 cases of moderate group and 33 cases of severe group according to the degree of liver injury.Another 50 healthy people who received physical examination in the hospital during the same period were selected as healthy control group.Liver function,HCMV-DNA level,serum specific HCMV-IgG and HCMV-IgM,and the relative expression levels of miRNA-9 and miRNA-122 a were detected in all subjects.The differences in expression levels of serum miRNA-9 and miRNA-122 a in patients with different degrees of liver injury and HCMV infection in NAFLD group were analyzed,and their correlation was further analyzed.RESULTS The levels of aspartate aminotransferase(AST),alanine aminotransferase(ALT),direct bilirubin(DBIL),total bilirubin(TBIL)and alkaline phosphatase(ALP),positive rates of HCMV-DNA,HCMV-IgG and HCMV-IgM and relative expression levels of serum miRNA-9 and miRNA-122 a in NAFLD group were significantly higher than those in healthy control group(P<0.05).The positive rate of HCMV-DNA,positive rate of HCMV-IgM and relative expression levels of serum miRNA-9 and miRNA-122 a in mild,moderate and severe groups were significantly increased in turn(P<0.05).There was no significant difference in the positive rate of HCMV-IgG among the three groups.Spearman correlation analysis showed that serum miRNA-9 and miRNA-122 a levels in patients with NAFLD were positively correlated with the degree of liver injury(P<0.05).The relative expression levels of serum miRNA-9 and miRNA-122 a in patients with NAFLD complicated with HCMV infection were significantly higher than those in uninfected patients(P<0.05).The expre
