摘要
目的 探究高危型人乳头状瘤病毒(HPV)感染及HPV-DNA载量水平对不同程度宫颈病变患者宫颈癌相关增殖基因表达水平的影响。方法 选取2018年12月-2020年12月于海口市人民医院治疗的86例宫颈癌患者、79例宫颈上皮内瘤变(CIN)患者及80例慢性宫颈炎患者为研究对象,采用杂交捕获Ⅱ代技术检测HPV DNA载量,免疫组化法检测各宫颈组织中B细胞特异性莫洛尼白血病毒插入位点1(BMI-1)、原癌基因C-myc的表达,比较不同程度宫颈病变患者高危型HPV感染情况和BMI-1、C-myc的表达情况,分析高危型HPV感染对宫颈BMI-1、C-myc的影响。结果 宫颈癌组HPV感染率、HPV DNA载量、BMI-1及C-myc阳性表达率均高于CIN组和慢性宫颈炎组(P<0.05),CIN组HPV感染率、HPV DNA载量、BMI-1及C-myc阳性表达率均高于慢性宫颈炎组(P<0.05),CIN患者宫颈细胞中BMI-1、C-myc表达与CIN分级和高危型HPV感染有关(P<0.05),宫颈癌患者宫颈细胞中BMI-1、C-myc表达与分化程度、FIGO分期及高危型HPV感染有关(P<0.05),Pearson相关分析显示,宫颈癌患者宫颈细胞中BMI-1评分、C-myc评分与HPV DNA载量分别呈两两正相关(P<0.05)。结论 合并高危型HPV感染的宫颈癌患者中,宫颈细胞BMI-1、C-myc高表达,其相互作用可能在宫颈癌的增殖中占据重要位置。
OBJECTIVE To explore the effect of high-risk human papillomavirus(HPV) infection and HPV-DNA load on the expression levels of cervical cancer-related proliferation genes in patients with different degrees of cervical lesions. METHODS A total of 86 patients with cervical cancer, 79 patients with cervical intraepithelial neoplasia(CIN) and 80 patients with chronic cervicitis admitted to Haikou People′s Hospital from Dec 2018 to Dec 2020 were enrolled as the research objects. HPV DNA load was detected by second-generation hybrid capture. The expressions of B cell specific Morony leukemia virus insertion site-1(BMI-1) and proto-oncogene C-myc in cervical tissues were detected by immunohistochemistry. The high-risk HPV infection, and the expressions of BMI-1 and C-myc were compared among patients with different degrees of cervical lesions. The effect of high-risk HPV infection on cervical BMI-1 and C-myc were analyzed. RESULTS HPV infection rate, HPV DNA load, positive expression rates of BMI-1 and C-myc were the highest in the cervical cancer group, followed by the CIN group and chronic cervicitis group(P<0.05). The expression of BMI-1 and C-myc in cervical cells of CIN patients were related to CIN grading and high-risk HPV infection(P<0.05). The expressions of BMI-1 and C-myc in cervical cells of cervical cancer patients were related to differentiation degree, FIGO staging and high-risk HPV infection(P<0.05). Pearson correlation analysis showed that scores of BMI-1 and C-myc were positively correlated with HPV DNA load in the cervical cancer patients(P<0.05). CONCLUSION In the cervical cancer patients with high-risk HPV infection, BMI-1 and C-myc in cervical cells are highly expressed. And the interaction between them may play important roles in the proliferation of cervical cancer.
