摘要
目的分析腹腔镜辅助远端胃癌根治术后并发症发生的相关危险因素。方法选取86例行腹腔镜辅助远端胃癌根治术患者作为研究对象。收集患者性别、年龄、吸烟史、手术时间、合并症、肿瘤T分期、组织分型、分化程度、消化道重建方式信息,分析腹腔镜辅助远端胃癌根治术后并发症发生的影响因素。结果86例患者术后发生并发症12例,发生率13.95%(12/86)。单因素分析结果显示,合并症、组织分型、分化程度、消化道重建方式是腹腔镜辅助远端胃癌根治术后并发症发生的影响因素(P<0.05);logistic回归分析显示,存在合并症、胃腺癌、肿瘤低/中分化、BillroNhⅡ式吻合术是腹腔镜辅助远端胃癌根治术后并发症发生的危险因素(P<0.05)。结论存在合并症、胃腺癌、肿瘤低/中分化、BillroNhⅡ式吻合术是腹腔镜辅助远端胃癌根治术后并发症发生的危险因素,临床可据此进行针对性防治,以降低并发症发生率。
Objective To analyze the risk factors of complications after laparoscopic assisted remote gastric cancer.Methods 86 laparoscopic-assisted radical gastric patients were selected.Using our homemade influencing factors of laparoscopic assisted distal gastric cancer radical complications questionnaire collected patients'gender,age,smoking,history,surgery time,complications,tumor T stage,tissue classification,differentiation degree,digestive tract reconstruction information,analyze the influencing factors of laparoscopic assisted radical remote gastric cancer complications.Results Of 86 patients,12 cases had complications,13.95%(12/86).Single factor analysis showed that complications,tissue classification,differentiation,digestive tract reconstruction were factors affecting the complications of laparoscopic assisted remote gastric cancer(P<0.05);logistic regression analysis showed that complications,gastric adenocarcinoma,low/middle differentiation,BillroNhⅡanastomosis were risk factors for complications after laparoscopic assisted remote gastric cancer(P<0.05).Conclusion Amorbidities,gastric adenocarcinoma,low/middle differentiation and BillroNhⅡanastomosis are risk factors for laparoscopic assisted remote gastric cancer.We can evaluate the risk of postoperative complications and control the results to reduce the incidence of complications.
