3D打印技术辅助胸腔镜肋骨骨折反向内固定的临床研究 被引量:2

摘要 目的 探讨3D打印技术辅助胸腔镜下肋骨骨折微创反向内固定的临床应用价值。方法 随机选取并回顾性研究2018年至2021年于我院进行肋骨骨折手术治疗患者85例。研究组患者47例,采用3D打印技术辅助胸腔镜下肋骨骨折微创反向内固定治疗;对照组患者38例采用传统开胸术钛镍合金肋骨接骨板治疗。比较两组患者手术效果,呼吸功能及术后疼痛程度。结果 两组护理前肺功能及血气分析指标无统计学差异(P> 0.05);护理后研究组肺功能、血气分析指标较护理前明显改善,而且优于对照组(P <0.05)。研究组理想镇痛率明显优于对照组(P <0.05),研究组有效率高于对照组,差异有显著性(P <0.05)。结论 结论3D打印技术辅助胸腔镜下肋骨骨折微创反向内固定可明显缓解疼痛、术中出血量少,降低肺部感染发生率。根据打印的肋骨模型挑选合适的反向环抱式肋骨记忆合金固定器做手术设计,优点之一是避免了因为不合适重复更换内置物的情况,缩短手术时间,能使骨折特别是粉碎性骨折解剖对位,内固定坚强,创伤小,患者术后1 d可下床活动,早期拔管,术后呼吸功能恢复快,住院时间短等优点,减轻了护理工作量,并发症少。该方法值得在临床推广。 Objective To explore the clinical value of 3D printing assisted thoracoscopic minimally invasive reverse fixation of rib fracture.Methods Eighty-five patients with rib fracture who underwent surgical treatment in our hospital from 2018 to 2021 were randomly selected and retrospectively studied.47 patients in the study group were treated with 3D printing assisted thoracoscopic minimally invasive reverse internal fixation of rib fractures;38 patients in the control group were treated with titanium nickel alloy rib plate after traditional thoracotomy.The surgical effect,respiratory function and postoperative pain were compared between the two groups.Results The indexes of lung function and blood gas analysis before nursing were the same in the two groups(P>0.05);After nursing,the indexes of pulmonary function and blood gas analysis in the study group were significantly improved compared with those before nursing,which was better than that in the control group(P<0.05).The ideal analgesic rate of the study group was significantly better than that of the control group(P<0.05),and the effective rate of the study group was significantly higher than that of the control group(P<0.05).Conclusion 3D printing assisted thoracoscopic minimally invasive reverse internal fixation of rib fracture can significantly alleviate pain,reduce intraoperative bleeding and reduce the incidence of pulmonary infection.According to the printed rib model,select the appropriate reverse encircling rib memory alloy fixator for surgical design,one of the advantages is that it avoids the repeated replacement of built-in objects due to inappropriate,shortens the operation time and enables the anatomical alignment of fractures,especially comminuted fractures,strong internal fixation,less trauma,patients can get out of bed one day after operation,early extubation,rapid recovery of postoperative respiratory function,short hospital stay and other advantages,reducing nursing workload and less complications.It is worthy of clinical promotion.
