摘要
目的探讨后筋膜囊下注射曲安奈德在白内障超声乳化术中对术后黄斑水肿的预防作用。方法选取茂名市电白区中医院2018年6月—2019年6月收治的白内障患者100例(100眼)为研究对象,按照随机数表法分为治疗组和对照组,每组50例(50眼)。对照组仅进行白内障超声乳化术,治疗组在对照组的基础上于术中在后筋膜囊下注射曲安奈德。通过记录两组患者术前术后的最佳矫正视力(BCVA)、黄斑中心凹厚度(CMT)、眼压(IOP)、不良反应发生情况分析两组患者治疗后的临床效果。结果观察组患者术后1周、1个月、3个月的BCVA分别为(0.62±0.24)、(0.61±0.25)、(0.60±0.23),显著优于对照组,差异有统计学意义(t=2.198、2.857、2.914,P<0.05);观察组患者术后1周、1个月、3个月时的CMT分别为(181±30)、(186±25)、(190±28)μm,显著低于对照组,差异有统计学意义(P<0.05);两组患者术前和术后的IOP比较,差异无统计学意义(P>0.05);两组患者术后不良反应发生情况比较,差异无统计学意义(P>0.05)。结论在后筋膜囊下注射曲安奈德能控制白内障超声乳化术后患者黄斑中心凹厚度,提升视力水平,具有好的临床疗效。
Objective To investigate the preventive effect of triamcinolone acetonide injection under posterior fascia on postoperative macular edema during cataract phacoemulsification.Methods A total of 100 cataract patients(100 eyes)admitted to Dianbai District Hospital of Tradi⁃tional Chinese Medicine from June 2018 to June 2019 were selected and divided into a treatment group and a control group according to the random number table method,with 50 cases(50 eyes)in each group.The control group only underwent cataract phacoemulsification,and the treatment group was injected with triamcinolone acetonide under the posterior fascia during the operation on the basis of the control group.By recording the best corrected visual acuity(BCVA),foveal thickness(CMT),intraocular pressure(IOP),and adverse reactions before and after surgery in the two groups of patients,the clinical effects of the two groups of patients after treatment were analyzed.Results The BCVA of the observation group at one week,one month and three months after the operation were(0.62±0.24),(0.61±0.25)and(0.60±0.23),which were significantly better than those of the control group,and the difference was statistically significant(t=2.198,2.857,2.914,P<0.05).The CMT of the observation group at one week,one month,and three months after the operation were(181±30)μm,(186±25)μm and(190±28)μm,which were significantly lower than those of the control group,and the difference was statistically significant(P<0.05).There was no statisti⁃cally significant difference in IOP between the two groups before and after surgery(P>0.05).There was no statistically significant difference in the incidence of postoperative adverse reactions between the two groups(P>0.05).Conclusion The injection of triamcinolone acetonide un⁃der the posterior fascia can control the foveal thickness and improve the visual acuity of patients after cataract phacoemulsification,which has a good clinical effect.
