摘要
目的分析在未破裂异位妊娠患者中采取经阴道超声引导穿刺介入术的应用效果。方法回顾性选取2018年3月—2020年3月淄博市妇幼保健院收治的140例未破裂异位妊娠患者作为本研究对象,根据不同方法分成参照组与研究组,各70例。参照组患者采取肌内注射甲氨蝶呤治疗,研究组患者采取经阴道超声引导穿刺介入治疗。比较两组治疗效果、输卵管再通情况、再次妊娠情况及各项生化指标。结果研究组治疗总有效率98.57%高于参照组85.71%,差异有统计学意义(P<0.05)。治疗后,研究组患者血β-绒毛膜促性腺激素(β-HCG)下降至正常范围所需时间、治疗后7 d包块直径及住院时间均短于参照组患者,差异有统计学意义(P<0.05),两组月经复潮时间对比,差异无统计学意义(P>0.05)。研究组输卵管再通率70.00%高于参照组51.43%,差异有统计学意义(P<0.05)。研究组再次异位妊娠率7.14%低于参照组25.71%,差异有统计学意义(P<0.05)。研究组足月分娩率78.57%高于参照组58.57%,差异有统计学意义(P<0.05)。结论在未破裂异位妊娠患者中采用经阴道超声引导下穿刺介入治疗,能有效提高患者的治疗效果、输卵管再通率,改善患者的妊娠情况,降低患者住院时间,加快康复时间。
Objective To analyze the application effect of transvaginal ultrasound-guided puncture intervention in patients with unruptured ecto⁃pic pregnancy.Methods A total of 140 patients with unruptured ectopic pregnancy who were admitted to Zibo Maternal and Child Health Hos⁃pital from March 2018 to March 2020 were retrospectively selected as the research subjects,and divided into a reference group and study group according to different methods,with 70 cases in each.The patients in the reference group were treated with intramuscular injection of methotrexate,and the patients in the study group were treated with transvaginal ultrasound-guided puncture intervention.The treatment ef⁃fect,fallopian tube recanalization,re-pregnancy and various biochemical indicators were compared between the two groups.Results The total effective rate of the study group was 98.57%higher than that of the reference group 85.71%,the difference was statistically significant(P<0.05).After treatment,the time required for bloodβ-chorionic gonadotropin(β-HCG)to drop to the normal range,the mass diameter 7 days after treatment and the length of hospital stay in the study group were all shorter than those in the control group,and the differences were sta⁃tistically significant(P<0.05),there was no statistically significant difference in the time of menstrual rehydration between the two groups(P>0.05).The recanalization rate of fallopian tubes in the study group was 70.00%higher than that in the reference group(51.43%),and the dif⁃ference was statistically significant(P<0.05).The recurrence rate of ectopic pregnancy in the study group was 7.14%lower than that in the reference group(25.71%),and the difference was statistically significant(P<0.05).The full-term delivery rate in the study group was 78.57%higher than that in the reference group(58.57%),and the difference was statistically significant(P<0.05).Conclusion In patients with unrup⁃tured ectopic pregnancy,the use of transvaginal ultrasound-guided puncture interventional therapy can
